超高齢社会を背景に増加する左室駆出率保持型心不全(HFpEF)は、未だ治療法が確立されていないため、発症ならびに重症化の予防が重要である。このためにはHFpEF発症の高リスク例や治療抵抗性前の軽症HFpEFと言った早期HFpEFを検出できる手法の確立が必要である。しかし、本邦のHFpEFは高齢者が主体で、肥満の多い欧米のHFpEFとは明らかに臨床像が異なっているものの、診断基準は欧米から提唱された方法を使用している。そこで、本研究は日本人のHFpEFを正確に診断するために日本人のデータからHFpEFの診断基準を策定することを目標にしている。精度の高い診断手法を確立するには多数例から得た包括的な心エコー図データをはじめとする臨床データが必要であり、本研究は前向き多施設共同研究として計画された。2022年度内に国内30施設の参加が確定し、多施設共同研究開始に向けたシステムの構築を行なっている。デジタル化を積極的に取り入れた症例登録システム構築がほぼ終了し、2023年度前半には症例登録を開始できる見込みである。全体で800症例の登録が見込まれている。これまで本邦の患者データに基づいたHFpEF診断基準は国内にはなく、早期HFpEFの診断に着目した研究は国際的に見ても皆無である。以上の点において本研究の新規性があると考えられる。研究から早期HFpEFの診断手法を確立できれば、HFpEFの発症ならびに重症化の予防に貢献できると考えられる。
