项目摘要

近年本邦は、高齢化社会の影響と相まって、心不全患者が増加の一途をたどっている (心不全パンデミック)。それら増加している心不全患者の予後は、新規心不全の治療薬の登場で今後改善する可能性もあるが、現状ではいまだ不良である。一方、心不全患者における口腔機能に関しては報告がないが、実臨床では心不全患者の口腔機能低下例をしばしば経験する。本研究では、心不全患者に合併する口腔機能低下例に対し、歯科医師による専門的口腔ケアで介入することにより、心不全患者の予後を改善することができるか、という仮説を検証するものである。当初、主要評価項目を心不全入院イベントに設定したが、本研究は当院のみの単施設介入研究であり、症例数を考慮し、主要評価項目を、BNPの変化率に変更し、心不全イベントは副評価項目に変更した。また、専門的口腔ケア群の研究対象者の時間的負担軽減を考慮し、専門的口腔ケアの受診回数は、当初の毎月1回から、口腔内の状態に応じて1～3か月に1回、に変更した。さらに、通院にかかる費用は研究費負担とすることで、対象者の経済的負担も軽減させるように変更した。上記変更点に沿って、現在対象患者を選定中である。本研究において、心不全患者に対する専門的口腔ケアが心不全のバイオマーカーや予後を改善することが示されれば、本邦の心不全パンデミックを抑制できる可能性や、現在注目されている医科歯科連携による健康寿命延伸、医療費抑制の観点からも非常に重要な研究になると考えられる。
