当院で胃癌に対して、術前化学療法施行例、幽門狭窄症例を除外した根治的幽門側胃切除術を施行した症例に対して、術前と術後半年後の胃液ならびに唾液を採取した。術前術後と現時点で71症例(BillrothⅠ法再建40例　Roux-en-Y再建31例)のそれぞれのサンプルを採取し、大阪大学微生物病研究所内にある株式会社ビケンバイオミクスに依頼しNGS解析を行った。メタゲノム解析により得られた細菌情報をもとにOperational taxonomic unit (OTU) 解析と、shanonn index解析、LEｆSe解析、UniFrac解析を行い、術前後の胃内細菌叢の変化と再建法別による胃内細菌叢の変化を解析した。幽門側胃切除術術前後で、OTUs解析、Shnnon index解析にてα多様性に有意差を認めた。またUniFrac解析にてもβ多様性に有意差が認められた。再建法別に解析したところ、α多様性、β多様性ともに有意差は認められなかった。症例を集めつつ、幽門側胃切除術前後の口腔内細菌叢のメタゲノム解析とバイオインフォマティックス解析を検討している。また幽門側胃切除術半年後の、体重変化量、血中総蛋白・アルブミン・リンパ球数を測定する。

For gastric cancer, preoperative chemotherapy, pyloric stenosis, radical pyloric gastrectomy, gastric juice and saliva were taken before and six months after surgery. 71 cases (Billroth I reconstruction 40 cases Roux-en-Y reconstruction 31 cases) were analyzed by NGS in Osaka University Institute of Microbiology. The analysis of bacterial information obtained from the operation was performed by Operational Taxonomic Unit (OTU) analysis, Shanon index analysis, LEfSe analysis, UniFrac analysis, and reconstruction of gastric bacterial flora before and after operation. Before and after pyloric lateral gastrectomy, OTUs analysis, Shnon index analysis, α diversity, intentional difference recognition UniFrac analysis, beta diversity, intentional difference, recognition, etc. The reconstruction method is different from the analysis, α diversity, β diversity and intentional difference. The analysis of bacterial flora in oral cavity before and after pyloric gastrectomy Six months after pyloric gastrectomy, body weight and serum albumin were measured.
