腎癌を含む多くの癌で肥満が発症のリスクファクターとなることが明らかとなっている。しかし肥満が腎癌の腫瘍学的転帰の改善にも関与することが報告されており、‘obesity paradox’と呼ばれているがその機序は不明である。腎臓は腎周囲脂肪に覆われており、脂肪組織には白色脂肪組織と褐色脂肪組織が存在しアディポサイトカインを分泌しているが、腎癌への直接的な影響は明らかにされていない。そこで今回我々は腎周囲脂肪が腎癌の発育にどのような影響を及ぼすのか、また腎癌治療に対してどのような影響を与えているのか詳しく検討するため本研究を立案した。初年度としては、腎周囲脂肪の採取は順調に行えており、サンプルはあるので引き続き研究を進めていく予定である。今後は採取した腎周囲脂肪検体を用いてadipocytokine関連（adiponectin、adiponectin receptor(Adipo-R1、Adipo-R2)、 leptin、leptin receptor(Ob-R)、visfatin, apelin、Apelin receptor(APLNR)、MMP14、UCP1、ANGPTL1-4）の免疫染色を行い、病理学的悪性度、progression free survival、overall survivalとの検討を行う予定である。また、同サンプルからRNAを抽出してQPCR法にてadipocytokine（adiponectin、 leptin、visfatin、apelin、MMP14、UCP1、ANGPTL1-4）の測定も行う予定である。

Kidney cancer contains multiple types of cancer. The mechanism for the improvement of kidney cancer pathology is unclear. The kidney is surrounded by fat, subcutaneous fat, white adipose tissue and brown subcutaneous fat. The direct effect of kidney cancer is obvious. This study was conducted to investigate the effects of perirenal fat on kidney cancer development and treatment. In the beginning of the year, the fat around the kidney was taken in order to study the progress of the study. In the future, the immunostaining of adipocytokine (adiponectin, adiponectin receptor(Adipo-R1, Adipo-R2), leptin, leptin receptor(Ob-R), visfatin, apelin, Apelin receptor(APLNR), MMP 1 - 4, UCP1, ANGPTL1-4), pathological degree, progression free survival, and overall survival will be determined. QPCR was used to detect adipocytokine (adiponectin, leptin, visfatin, apelin, MMP 1 - 4, UCP1, ANGPTL1-4).
