高齢者や嚥下障害者の咳テストの結果は無症候性脳梗塞（SCI）に影響を受けている可能性があるため、我々は不顕性誤嚥のスクリーニングテストとして汎用されているクエン酸を用いた咳テストを用いて、一般的な若年者や高齢者および嚥下障害者において咳テストを実施し、結果がrisk factor of SCIの影響を受けるかを検討した。対象者は嚥下障害のない若年者（21名：男性5名　女性16名　平均年齢32歳）、高齢者（18名：男性7名　女性11名　平均年齢78歳）と嚥下障害者（16名：男性10名　女性6名　平均年齢75歳）であった。若年者群には高血圧症1名、高齢者群には心疾患6名、糖尿病4名、脳血管障害3名、認知症3名、高血圧症2名、嚥下障害者群には口腔がん9名、脳卒中2名、高血圧症1名、脳腫瘍1名　胃がん1名、心疾患1名、パーキンソン病1名がいた。。対象者に1％クエン酸生理食塩水をネブライザより噴霧し、1分間吸入させ咳反射を誘発させた。結果として、若年者群は全員咳反射がみられたが、高齢者群は50％、嚥下障害者群は80％であった。若年者群と高齢者群間のみ差がみられた。また初回の咳までの時間は若年者群が高齢者群および嚥下障害者群より有意に早かった。1分間当たりの咳の回数も若年者群が高齢者群や嚥下障害者群より有意に多かった。咳の誘発時間や回数について、年齢、服薬数、咽頭期嚥下障害、risk factor of SCIで調整を行ったところ、risk factor of SCIと年齢が大きく影響を及ぼしている因子であった。また、ロジスティック回帰分析の結果、咳反射の有無についてrisk factor of SCIに有意なオッズ比を認めた。以上より、不顕性誤嚥のスクリーニングテストとして汎用されている咳テストは、risk factor of SCI存在が影響していることが示唆された。
