認知症高齢者が、身体疾患の治療目的で急性期病院に入院することが増加している。認知機能の低下による混乱や理解不足を原因とする入院・治療への拒否、治療妨害などの危険行為が対応困難な深刻な問題となっている。急性期病院の使命である救命や治療の安全遂行のために、薬剤や身体拘束等に頼らざるを得ないのが現状で、その結果、高齢者の生きる意欲の低下や廃用症候の進展等の深刻な弊害が生じている。そこで、市内2か所の急性期病院の病棟看護師・認定/専門看護師との協働で、急性期病院での認知症高齢者の尊厳保持と治療遂行を可能にする看護実践モデルの開発を目的とし、初年度の取り組みとして「超急性期における認知症高齢者への看護実践の可視化」に取り組んだ。具体的には救命・治療に重点を置く「超急性期」において、どのような看護対応が認知症高齢者の安心・安定した「良い状態」を引きだし、廃用症候を最小限に抑え治療の安全遂行を可能にするのか、また、逆にどのような対応が混乱を増大させ暴力行為や治療拒否・妨害等に繋がるのか、研究者が看護ケア提供場面に観察者として参加して記録した。本年度は観察基準の策定、協力文書の作成を経て、協力病院への説明、協力内諾のち、大学および病院の倫理委員会の承認を得るのに時間を要した。そのため調査期間は2016年1月中旬～3月下旬の約2か月余りであったが、合計32日間、7名の看護師の協力を得て10名の高齢者の看護ケア提供の計93場面の記述記録の蓄積が終了した。「良い状態」を引きだす看護対応としては、「了解のサインを得るまで待つ」「丁寧な日常生活ケアの提供」「療養生活の非単調化」「ケア効果のフィードバック」などが明らかになった。今後、さらに分析を進展させ、看護師に求められる認知症高齢者の「良い状態」を引き出す対応の明確化を進展させ、臨床活用可能モデルの提示へとつなげたい。

The number of hospitalizations for acute cognitive disorders and physical disorders has increased. Low cognitive functioning, confusion, lack of understanding, hospitalization, treatment rejection, treatment impairment, risk behaviors, and profound problems. The mission of the hospital in the acute phase is to save lives, treat patients safely, and exercise physical restraint. In addition, the group of nursing practice for patients with hypercognitive disorder in acute phase hospital, identification/department nursing staff, and possible implementation of treatment for patients with hypercognitive disorder in acute phase hospital was selected from the group of "visualization of nursing practice for patients with hypercognitive disorder in hyperacute phase." Specific focus on life-saving treatment: hyperacute phase, care, comfort, stability, and good state for those with cognitive disorders, use of symptoms, minimization of symptoms, and safe management of treatment. The investigator provided the scene and the observer participated in the recording. This year, we will review the policy setting of the benchmark, the preparation of the cooperation document, the explanation of the cooperation hospital, the cooperation within the hospital, and the recognition of the ethics committee of the university. During the investigation period, from mid-January to late March, 2016, about 2 months later, a total of 32 days, 7 nurses worked together to obtain 10 nurses, and 93 scenes were recorded. "Good state" refers to nursing care,"understanding of service,""daily life,""non-uniform adjustment of recuperation life,""failure of results," and "waiting for service." In the future, the analysis progress, the nurse seeks to clarify the "good state" of the cognitive disorder, and the clinical application of the possible "good state".
