项目摘要

本研究では、認知症高齢者の代理決定に関する日本における研究動向と課題の整理（課題Ⅰ）、看護師の代理決定支援と看護支援に対する家族の受け止め・評価の相違（課題Ⅱ）、代理決定に対する遺族の満足度に関連する要因（課題Ⅲ）の3課題の解明を通じて、意思確認が困難な認知症高齢者の終末期医療に関する家族の代理決定を支える看護のあり方について検討・提言する。課題Ⅱは、遺族・看護師ペアに対して行った面接調査の逐語録を素材に、M-GTAを用いて分析を進めた。課題Ⅲは、次のステップで研究を進めた。第１ステップでは、いつの時期に代理決定した遺族を対象とするか決定した。課題Ⅱの結果、代理決定から看取りまでの期間は1年以内が7割、2年以内が9割であった。この調査では代理決定後看取りまでに2年間必要であったことから、調査対象とする遺族は平成26・27年度の2年間に代理決定した人と決定した。第2ステップでは、調査協力依頼をする施設の選定を行った。厚生労働省「介護サービス情報公表システム」に掲載されている中部地方の特別養護老人ホームのうち、主な介護報酬の加算「看取り介護の実施」として公表されている916施設を調査協力依頼の対象として選定した。第3ステップは、施設への協力依頼と、承諾が得られた施設に対する遺族票の送付と回収であった。遺族調査の協力が得られた施設は34施設であり、協力施設を通じて遺族に遺族票を配布した。3月末日現在、120名の遺族から回答を得ている。今年度は以下の補充調査も行った。課題Ⅰと課題Ⅱの分析を進めた結果、遺族調査の分析モデルに施設要因を追加する必要が出てきた。そのため、施設要因に関する情報を次の2つの方法で収集した。第１の方法は厚生労働省「介護サービス情報公表システム」からの収集であり、第2の方法は施設調査である。施設票は、162施設から回答を得た。
