项目摘要

1.目的本研究は、抗菌剤を添加した仮着材を暫間被覆冠の仮着に応用し、仮着という短い作用期間で支台歯のう蝕病巣を殺菌する方法を考案し、補綴治療へ導入することを目的としている。昨年度までの研究で、混合抗菌剤(Metronidazole、Ciprofloxacin、Cefaclor、各2% w/w)を添加した仮着材がう蝕病巣細菌の殺菌に有効であることがin vitroで確認された。今年度は臨床治験を行い、本術式の臨床応用に対する評価を行った。2.方法本学歯学部附属病院補綴科を受診した患者のうち、象牙質に達するう蝕のある生活歯で、補綴処置が必要と診断された歯を有する患者6名を対象とした。本研究に関して十分な説明を行い、同意を得た上で、対象歯に混合抗菌剤添加仮着材を応用し、仮着の前後でう蝕病巣を採取し、試料中の細菌数から細菌効果を判定した。さらに、臨床症状を診査し、被験歯および歯周組織への影響を検討した。3.結果術前のう蝕病巣からは10^5CFU/mg以上の細菌が検出された。一方、術後では仮着期間が1〜2週間であった5例からは細菌の検出を認めず、う蝕病巣が無菌化されていた。仮着が3週間に及んだ1例では、辺縁漏洩が認められ、10^3CFU/mgの細菌が検出された。いずれの症例においても、歯髄炎や歯周炎などの臨床症状は認めなかった。4.考察以上の結果より、本術式が臨床応用可能であることが明らかになった。また、その際には仮着期間を1〜2週以内とし、辺縁漏洩への配慮が必要であることも示唆された。この研究成果をHori,R,Kohno,S,Hoshino,E.:Bactericidal eradication from carious lesions of prepared abutments by an antibacterial temporary cement.J Prosthet Dent 77:348-352,1997に発表した。

1. Objective: To investigate the method of adding antibacterial agents to the implant and to introduce the patch therapy into the implant. In the past year's research, mixed antibacterial agents (Metronidazole, Ciprofloxacin, Cefaclor, 2% w/w each) were added to the materials to corrode the pathogenic bacteria. This year's clinical treatment is in progress, and the clinical application of this procedure is evaluated. 2. Methods Six patients were diagnosed in the Department of Prosthodontics of the Affiliated Hospital of the Faculty of Medicine. This study was conducted to determine the number of bacteria in the sample and the effect of bacteria on the sample before and after the addition of antibacterial agents. The clinical symptoms were examined, and the effects of oral and peripheral tissues were discussed. 3. Results Before operation, the number of bacteria above 10^5 CFU/mg was detected. 1~2 weeks after surgery, 5 cases were sterilized. The bacteria were detected in 3 weeks and 1 case respectively, and 10^3 CFU/mg was detected. The clinical symptoms of dental caries and periodontitis are recognized. 4.
