项目摘要

高齢者が緊急入院した際に、入院の契機となった骨折や熱中症など外科的・内科的疾患が治癒したにも関わらず、入院そのものが原因で自立度が低下する場合がある。これは入院関連機能障害と呼ばれ、特に認知症高齢者においては環境の変化に対する脆弱性のため入院関連機能障害の発症が一般高齢者と比較し高いことが知られている。本研究では認知症高齢者の入院関連機能障害に着目し、認知症高齢者の心身機能、日常生活機能、また居住形態などの社会的背景から入院機能障害の発症に強く関連している要因を明らかにすることを目指す。そこで本年度は、まず入院関連機能障害に関する先行研究のレビューを通じて関連領域の知見に関して整理を行い、入院関連機能障害を定量的に評価するための尺度の考案に焦点をあてて研究を進めた。認知症高齢者に対する入院関連機能障害に関しては日常生活動作に着目した評価が主流であったことから、入院前および入院後の日常生活動作自立度(ADL)得点の差分を軸とした質問紙のパイロット版を作成した。これについて関係者と共有の上、総合病院にて内科的疾患を理由に入院する患者に対して質問紙を用いた評価の実施について議論を行なった。並行して、地域在住認知症高齢者が入院を機に自立度を低下させるプロセスについて、個々の対象者に評価を行い経過を編纂した。これにより、入院の契機となる疾病・受傷を経験する前の日常生活自立度がその後の入院関連機能障害の発生に大きな影響を与えていることを臨床的事象として改めて確認した。また、副次的取り組みとして、①『高齢者の主観的健康観に基づく新たな老年学的概念に関する研究』、②『認知症カフェと医療者(医療従事者、医療機関)の連携に関する研究』に携わった。いずれも査読論文として国際誌に掲載された(①PLoS ONE誌, ②Journal of Alzheimer's diseases誌)

High fever patients are admitted to the hospital urgently, the opportunity of admission, fracture fever, surgery, internal medicine, the cause of admission, and the degree of independence are low. For those with high levels of hospital-related impairment, especially cognitive impairment, vulnerability to environmental changes, and development of hospital-related impairment, there is a high level of cognitive impairment. This study indicates that the main causes of hospital-related dysfunction in cognitive hyperactivity are related to psychosomatic function, daily life function, and social background of hospital-related dysfunction in cognitive hyperactivity. This year, we will review and evaluate the relationship between hospital related functional impairment and related knowledge. Cognitive disorders are related to admission related functional impairment, and daily life activities are related to evaluation, mainstream, pre-admission, and post-admission ADL scores. The relationship between the patient and the patient is a common problem. The patient is admitted to the hospital for medical reasons. The patient is questioned and evaluated. In addition, patients with high levels of cognitive disorders in the region have low self-reliance and low self-esteem. The cause of admission, disease, injury, self-reliance before admission, and the development of hospital-related functional disorders are major factors affecting the clinical situation.
