本研究は電子化診療録を念頭において医療用語の標準化が可能かどうかを検討したものである.診療録には,コード化・数値化が困難なnarrative dataが含まれ,この文書情報の管理が重要となるが,単なるテキスト管理では検索が困難でデータベース化も非効率である.そこで,医療文書に含まれる用語を統一しておけば,その用語での検索を考慮するだけで管理可能となる.ただし,用語集の作成は膨大な労力を要し,またその完備性の検証も実運用を必要とすると考えられていた.そこで,当初は医療文書を全文検索エンジンで前処理しておき,そのトークンを検索対象とすることにより用語集の完備性を間接的に評価する方法を取った.その後,実時間で文書中の単語を直接に用語集と比較することが可能な状況となったため,最終的にはこちらを採用した.用語集は,当初使用を予定していた用語を包括している日本語版MedDRAをLLTとPTのレベルで使用した.その結果,実用的な実時間処理でLLTでは医療文書の用語の約8割がヒットし,PTでは6割がヒットした.文書の種類,執筆者により内容は異なるが,LLTはほぼ満足できる水準にあり,またヒットしない用語はおおむね具体的な内容を表現しないものあるいは抽象的な用語が主で,特異的な検索対象とはならないものであった.これらから,抽象概念を表現する医療用語の補助用語集を併用すれば,MedDRAは実用的な医療用語集であると判断された.補助用語集の完備,電子化診療録での用語管理の具体的な方法,医療文書の用語チェックについて,今後検討を加える.
