项目摘要

本研究は、急性期治療を担う一般病棟における高齢患者に対する身体拘束の低減に向けて、看護師の倫理的行動ならびに病棟全体の倫理的組織文化を“見える化”するシステムを組み込んだ臨床で活用可能な倫理教育プログラムを開発することを目的としている。令和４年度は、高齢者看護の倫理的課題をふまえた一般病棟の看護師に向けた自己倫理評価ツールの開発に向けた全国調査を行う前に高齢者看護に特化した倫理的行動の尺度項目の絞り込みを行うため、高齢者医療に取り組む病院に調査協力を依頼した。承諾が得られた５病院の看護師431名を対象に、研究者らが開発中の尺度を含む高齢者看護を念頭においた倫理的行動に関して、2022年５～６月に郵送法にて無記名自記式質問紙調査をし、統計学的ならびに質的分析を行った。194名より返送があった（回答率45.0%）。日常倫理や患者中心の意思決定の項目においては、他と比べて低く評価する傾向があった。そのため、病院に勤務する看護師は、高齢患者に対する日常的な支援や意思決定支援の場面で倫理的な行動をとりにくい傾向があると予測された。また、高齢者を看護する上で回答者自身が重視している倫理的行動に関して尺度項目との比較を行った結果、高齢者を看護する上で最も重視している倫理的行動として、３カテゴリが抽出された。開発中の尺度項目に不足していたのは、本人のペースに合わせる行動、相手の立場に立った行動、安心でき穏やかな生活の提供、苦痛・不安を回避・軽減する行動などであった。今後は因子分析の結果と併せて自己倫理評価ツールの構成や項目を再検討する必要があると考えられた。
