本研究は,申請者らが脳卒中後の手指麻痺の改善を目的として開発した手指リハビリテーション支援システム(Functional Trhabikitation Support System:Narem)の効果検証を行うことを目的とする.研究方法は,脳卒中者を対象とした無作為化比較試験を行うとともに,MRIを用いた脳内の構造的および機能的ネットワーク解析により,手指麻痺の回復に関わる神経メカニズムを解明していく.研究実施計画は,① 回復期・維持期脳卒中者30名程(回復期20名,維持期10名)を対象として無作為化比較試験によるNaremの効果検証を実施していく.② ①に並行して,脳卒中者30名程(回復期20名,維持期10名)を対象に,MRIを用いてNarem介入前後の拡散テンソル画像(DTI)と安静時機能画像(rsfMRI)を撮像し,脳の領域間ネットワークの構造的結合と機能的結合の変化を横断・縦断的に調査し,Naremによる手指麻痺の回復と神経ネットワークの関係性を解明していく.③ 手指麻痺に対する従来の治療法(CI療法、ミラー療法)単独での治療とNarem併用による治療効果の比較検証を行い,Naremを用いた手指麻痺に対する新たな治療法の確立を目指していく.2022年度の研究実績は,① 回復期8名,維持期30名の介入検証を実施.② 回復期0名,維持期0名(5月に3名撮影予定)の脳画像撮像を実施.③ 回復期1名,維持期7名の介入検証を実施.学会発表3演題(2演題発表済み・1演題発表予定),論文作成中.現時点での研究成果としては,維持期脳卒中者に対して,Naremによる手指トレーニングを行うことで,手指機能の改善や痙縮の軽減,生活場面での使用頻度の向上に繋がっており,Naremは脳卒中者の手指機能の回復を目的とした治療システムとして有効であることが示唆されている.
