高齢糖尿病患者はその病態や治療により重篤な病態との関わりが強く、予防がまず重要となる。なかでも骨格筋量および筋力低下により定義されるサルコペニアは、糖尿病と互いの発症リスクにおいて悪循環を形成していると考えられているが、糖尿病関連指標がサルコペニアの進展を予測し得るかは不明である。現行のサルコペニア判定における骨格筋量評価法としてCT やMRI などが標準とされるが、実臨床ではこれらを用いての測定は、簡便性、迅速性という観点からは適さない。我々は日本人高齢男性コホートにおいて、前脛骨筋のエコー評価がサルコペニアと関連し、エコーによる骨格筋の量的・質的評価の有用性を報告してきた。本研究の目的は、１．高齢者糖尿病に対する運動介入がサルコペニアを改善させるか明らかにすること。さらに、２．運動介入の効果を判定するバイオマーカーとしてエコーによる骨格筋評価が有用であるか明らかにすることである。老年医学におけるトピックであるフレイル、その中核であるサルコペニアは、糖尿病など生活習慣病の存在により、その病態が促進されることが臨床的検討から明らかにされつつある。この観点に基づき、運動および栄養介入の効果判定に身体機能評価および血液検査を施行した高齢者（65歳以上）のデータベースを用い、骨格筋エコーを用いた簡便なサルコペニア評価法およびフレイルまたはサルコペニア指標との関連について検討してきた。本年度は、サルコペニアと骨格筋エコーに関連する論文を筆頭著者として国際誌1本、共同著者として国内誌1本が採択された。また、筆頭演者として国内学会２件と国際学会1件、共同演者として国内学会1件と国際学会1件の成果を残した。本年度得られた成果は、エコーを用いた下腿筋評価の礎となり、高齢糖尿病患者に対する新たな運動療法の意義を示すことにおいて重要である。
