项目摘要

本研究の目的は、１）外来におけるがん治療に関連した認知機能障害の実態と、２）がん治療に関連した認知機能障害を招く要因を探索し、３）がん治療に関連した認知機能障害を自覚している患者の体験と求める看護援助を明らかにすることである。研究計画として初年度は、がん治療に関連した認知機能障害に関する文献検索を実施した上で、実際および要因について調査を行う予定であった。文献検索を進める中で、CRCIに関する国内論文が少なく、海外の論文を中心に文献検索を実施した。目的３）にある看護援助について文献検索を実施した結果、8件の論文を抽出した。この8件の論文によると、がん治療に関連した認知機能障害に対する患者への介入は、コンピューターを用いた脳トレーニングや代償的認知トレーニングなどの認知行動療法と、マインドフルネスや気功、音楽療法などの補完代替療法によるものであった。こうした認知行動療法や代替補完療法は、国内では看護師が提供することは実際にはあまり見られず、理学療法士や心理療法士による介入となることが多い。特に、代替補完療法は、文化や制度などわが国において提供するにはハードルが高く、また、介入を継続しなければ効果が得にくいなど課題も多いことが考えられた。これらの結果を文献レビューとしてまとめ、論文投稿を実施した。この論文は、がん治療に関連した認知機能障害を自覚している患者の求める看護援助を検討する上で、実際に介入されている援助をもとに検討することが可能となり、研究を進めていく上で必要な資料であると考える。
