本研究の目的は、集中治療室で浅い鎮静管理を受けて覚醒状態にある人工呼吸器装着患者に対し、ICU看護師が“患者の意思が反映された患者自身で行う主体的な日常生活動作（本研究ではセルフケアと定義）”を支援するための臨床判断指標を見出すことである。人工呼吸器装着患者が覚醒状態にあっても、対象患者の身体状況や治療・処置、安静制限などによって患者の意思で自由に行動することは難しく、その行動の殆どは看護師の判断に委ねられる。本研究の成果により、ICU看護師の適切な臨床判断を支持する具体的な“臨床判断指標”を可視化し、実践への応用に繋げることが期待される。当該年度は下記、2つの研究目的達成のための研究計画の再考および準備を行った。・研究1．量的研究手法を用いて、集中治療室でLight sedation(浅い鎮静)管理下にあり、“覚醒状態にある人工呼吸器装着患者”のセルフケア実施状況とそれに関係する患者条件となる臨床データを、電子カルテから収集・分析して明らかにする。・研究2．質的手研究法を用いて、上記の対象患者を看護するICU看護師の語りから、ICU看護師が認識するセルフケア実施を促進・阻害する要因を明らかにする。調査の準備として、国内外の文献を検討し、研究1のデータ取得項目および研究2のインタビューガイドを精選した。また、被験施設の選定および対象者のリクルート方法を検討・調整し、所属施設の倫理審査委員会の受審に向けて準備を進めている段階にある。

The purpose of this study was to provide support for clinical judgment indicators that reflect the patient's own daily activities (defined in this study) in the intensive care unit (ICU) in the context of sedation management and waking up of patients with artificial ventilators. Respirators are designed to keep the patient awake, to monitor the patient's physical condition, to treat, to control, to allow the patient's freedom of movement, to control, to control, to control. The results of this study are expected to support the appropriate clinical judgment of ICU nurses by visualizing and implementing specific clinical judgment indicators. Quantitative study methods: centralized treatment room, light sedation management,"awake state, artificial respirator installed patients","clinical status, electronic treatment","analysis". Study 2. Qualitative hand-study method was used to record ICU nurse's comments on patient care, and ICU nurse's understanding of the main causes of promoting and preventing the implementation of nursing care. Preparation of the survey, review of domestic and foreign literature, acquisition of research items for study 1 and selection of research items for study 2 To review and adjust the methodology for the selection and selection of the target, and to prepare for the trial of the Ethics Review Committee of the facility.
