本研究の目的は、出生後早期に非侵襲的呼吸補助療法を受ける早産児の状態悪化を予見する呼吸変動を検出し、医師-看護師間の臨床推論をつなぐ、Early Warning Score（以下、Preemie-EWS)を開発することである。2022年度は、研究の第一段階としてスコア指標の確定を目指した。具体的内容は以下の通りである。1.スコーピング・レビュー・プロトコルの作成（2022年4月～2023年3月）：スコア指標となる変数を選定するための文献検討の方法として、スコーピング・レビューを用いることを計画し、レビュー・プロトコルを作成した。プロトコル作成に際しては共同研究者３名の確認を受けた。また、学術雑誌へのプロトコル掲載を目指して、英文によるプロトコルの記述を進めた。今後は、プロトコルが採択されたら速やかに文献レビューを開始し、スコア指標となる変数を選定後、指標確定に向けた質問紙調査を実施する。2.フィールド調査（2022年11月）：フィールド調査として、新生児専門医のコンサルテーションを受けた。現在、診療の場で早産児の呼吸管理に用いられている非侵襲的呼吸補助療法の種類と特徴、医師と看護師の協働に求められることについて情報を得た。これらの情報をスコーピング・レビュー・プロトコルに反映させ、プロトコルを修正した。3.Family and Infant Neurodevelopmental Education（以下、FINE）の研修受講（2022年8～9月）：英国のFINE Neonatal UK Ltd.が提供する新生児看護のトレーニングであるFINE-1をオンラインで受講し、コースを修了した。FINE-1のトレーニングには、早産児の生理的状態の観察と評価など本研究課題と関連する重要な内容が含まれている。今回修得した知識と技術は、今後の研究計画の安全な遂行に繋げるために活用する。

The objectives of this study were to detect respiratory changes in premature infants affected by noninvasive respiratory assistance therapy in the early postnatal period, to develop clinical inferences between physicians and caregivers, and to develop an Early Warning Score (hereinafter, Preemie-EWS). The knowledge and technology acquired today will be used for the safe implementation of future research projects.
