母乳は乳児にとって最良の栄養であり、母乳育児は母子間の愛着形成に効果的でである。近年、世界各国が母乳育児推進を推進している中で、出産後すぐに十分な母乳分泌が得られず飢餓状態となり、新生児マススクリーニング検査で脂肪酸代謝異常症の偽陽性と判定される場合がある。再検査を伝えられた家族の精神的負担は大きいと推測される。本研究では、飢餓によるマススクリーニング偽陽性例の実態調査を行い、新生児期の飢餓状態がその後の発育に与える影響を明らかにするとともに、地域の多職種と連携した新生児期の適切な栄養管理・母乳育児の支援を構築することを目的とする。新生児マススクリーニングは公的事業であるため、大分県に本研究の許可を得たのち、大分大学の倫理委員会の承認を得て研究を行う。対象は大分県全域で新生児マススクリーニングにタンデムマス法が導入された2014年4月から2020年3月までの過去6年間で新生児期の飢餓に起因すると考えられる脂肪酸代謝異常症の偽陽性例とその同胞である。新生児マススクリーニング採血濾紙に記載されている採血時の児の体重や哺乳状況などの情報、精密検査を実施した医療機関の診療録、母子手帳に記載されている成長の記録、健診時の情報を後方視的に検討する。一方で母親の母乳育児に対する考え方などをアンケートや聞き取り調査を実施し、3歳時の発育（身長、体重）に対する飢餓の影響を症例の臨床背景とともに検討する。

This study was conducted to investigate the effects of neonatal hunger on postnatal development and to establish appropriate neonatal care and support. The new child, Masukura, has a public career, the approval of this research in Oita University, and the recognition of the Ethics Committee of Oita University. The results showed that there were a large number of false positive cases of abnormal fatty acid metabolism in neonatal hunger in the past 6 years from April 2014 to March 2020.
