島の退院支援を支える専門職活動の一つとして、A島の認知症初期集中支援チームの活動について把握した。インタビュー対象は、認知症初期集中支援チーム5名である。役割を果たすために活動で工夫していることついて面接調査を実施した。その結果、三つの活動のタイプが明らかになった。①地元出身者が実践する地域文化に根差した活動：「長い保健師活動を通して、役場職員や住民から、様子がおかしい人の情報が入り、自然と課全体でも共有できる。保健師が直接確認することもあれば、課の担当者が気にかけて見守ることで、様子の変わってしまった住民をスクリーニングしている。」などがあった。②島外出身者が地域文化を取り込んだ活動：「地元出身者はチーム会議で、認知症の疑いのある人の生活の様子や介入方法へのきめ細かい配慮を提案していたので、外来の未受診者リストから介入する際は、地元出身者からの情報収集を加えて働きかける方法を工夫する」などがあった。島外出身者は、地元出身者の活動から、「なじみの他者」がもつ事例情報の有用性を学び、自らの活動にも地元出身者を効果的に活用していた。③地元出身者と島外出身者が地域文化を取り込み協働した活動：「事例が地元出身の専門職に強硬的で、同時に県外者には遠慮があるなどの個性があるときは、看護師であることと島外出身者であることが事例に伝わるよう強調しながら、地元出身保健師の関係の限界を支える」など相互補完的な介入も行っていた。A島の認知症初期集中支援チームの活動には、地域文化が活かされていた。生活を共有してきた「なじみの他者」という、当事者の“つながり”を支援に取り入れ、個々のアウトリーチ力、チームのアウトリーチ力を強化していた。
