DemDx will deliver an urgently needed Care Home focused Clinical Assessment Tool (CHCAT).The need is driven by the introduction of the Enhanced Health in Care Homes initiative-aimed at providing a more proactive health service to the elderly and frail care home residents by increasing frontline clinical support to care home nursing staff, community rapid response teams led primarily by Allied Healthcare Professionals (AHPs).Care home nursing staff and communities teams require expanded generalist knowledge across hundreds of conditions, signs, symptoms and "red flags," recognition of patient-specific comorbidities, and awareness of frequently updated gold standard protocols. These clinicians are urgently calling for tools to support them in their expanded roles.Currently both community and care home clinical teams rely heavily on clinical guidance from GP surgeries (with knock on impact on GP workloads and waiting lists) or over-refer patients to A&E, putting further pressure on acute care pathways including ambulance call outs..CHCAT builds upon DemDx's sector-leading clinical reasoning intelligent support platform (CRISP), co-developed via InnovateUK and NIHR-funded engagement in five NHS Trusts. It is the gold standard in support tools for allied health professionals.CHCAT will be a revolutionary cloud-based clinical assessment tool, using AI and data to support community/care home staff in better, safer, quicker clinical assessment and treatment or referral decisions.Innovations include:* AI allowing dynamic consideration of complex patterns and associations across 10,000s of symptoms, comorbidities and "red flags" in validated NICE data.* A data-driven structure enabling AHPs to make sense of complex data associations to make safe, consistent decisions, with patient-centric data weighting enabling prioritisation of most probable conditions, to inform best referral options* Powered by a 12m-patient CPRD dataset.* Outputs are tailored to ageing-specific clinical assessment pathways for the 5+ most common ailments* CHCAT is fully interoperable, seamlessly integrated into patient primary care/acute pathways.
