What works to support the mental health of young people leaving public care in Wales?

Care leavers have high rates of mental ill health and the transition from care significantly impacts mental health. Improving long-term health and social outcomes for care leavers will critically depend on models of transition that support their mental health at the pivotal time of transition from care to independence. Despite a proliferation of models of care transition, there is little understanding about what works. This mixed method realist evaluation will investigate what works about transitional care to support care leaver mental health, for whom, and under which circumstances and which transitional models of care in Wales represent 'best practice'.
