This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. This study will determine if pioglitazone, currently approved for the treatment of type ii diabetes by reducing resistance, will reduce the overall risk for stroke and myocardial infarction in non-diabetic patients. Patients over the age of 44 years, who have had a recent ischemic stroke, will be enrolled if they have insulin resistance as tested per protocol. Patients who qualify will be on a doulbe blind study getting either placebo or pioglitazone for a total period of three years on the drug
