This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. This study will examine the clinical efficacy of a newly developed flowable composite resin for the restoration of small occlusal caries occlusal lesions. Small carious lesion occlusal lesions will be restored with one of two flowable composite resins. One Tetric-Flow is a well-recognized composite resin with decreased filler loading to improve adaptation. The other is a new material from L. D. Caulk. These materials will be placed in two occlusal cavity preparations in the same patient and evaluated for wear, retention, color match, marginal adaptation, anatomic form, marginal discoloration, axial contour, and secondary caries. Post-operative Sensitivity will be measured using a Visual Analog Scale and Gingival Condition will be assessed using the Gingival Index of Loe and Silness. Thirty class 1 restorations of each material for a total of 60 restorations will be inserted. The lesions will be prepared and caries removed, with rubber dam isolation. All bonding will be done using each manufacturer's bonding agent to moist surfaces. The two preparations will receive the different composites and different adhesive techniques to evaluate those systems. The restoration will be placed in one or two increments with each increment cured for 20 seconds with a visible light-curing unit. Carbide finishing burs will be used to remove gross excess, followed by finishing strips and disks (Enhance Polishing System). Each restoration will be evaluated directly and indirectly at baseline (one week after restoration placement), 3 months, 6 months, and one year. Using calibrated examiners, each restoration will be evaluated for marginal discoloration, secondary caries, loss of anatomical form, retention, surface texture, and marginal adaption. Slides will document marginal staining of the composite restoration. Impressions will be made at each recall and used to determine wear of the restorations. Groups will be compared annually using an ANOVA for Repeated Measures with the level of significance set at .05.
