Hybrid E-zine Health Education intervention integrated within a Mental Health package for Adolescent Perinatal Depression (E-zineMH)

Abstract High temperatures in tropical regions could increase mental and physical health complications of perinatal adolescents and their infants, increasing their risks of depression and adverse obstetric complications such as miscarriages, preterm birth, low infant birth weight and still births. The United Nations recommend health education integrated within collaborative health packages in antenatal clinics to address the impact of climate change. Barriers to implementing environmental health care for the perinatal population as part of integrated packages in primary care in Nigeria are high at the needs (context-relevant), service(capacity of providers), and consumer awareness (uptake of services) levels. This application builds on the evidence and lessons learned from previous work on perinatal adolescents and ongoing K43 award PI (Dr. Kola's-K43TW011046-4) in Nigeria on, mhGAP -perinatal depression integrated model within primary care, mHealth and technology strategy testing, and implementation system development research. As part of a K43 emerging global leader award, we designed and developed a self-management mobile phone application as an adjunct treatment and part of a hybrid (face-to-face + mobile phone follow-up care) intervention for perinatal adolescents to promote positive mental and physical health behavior using age and culturally appropriate approaches. The perinatal adolescents who participated in the trial and their caregivers so far have reported high levels of satisfaction with the intervention. Our experiences identified the vital role of family members and other caregivers (who are often too busy to regularly accompany their wards to the clinics on antenatal days) in service designs for this population. We are now proposing to further leverage virtual space and vital role of family members to deliver a Hybrid E-zine health education intervention integrated within a Mental Health package for perinatal depression ( E-zineMH) , to minimise the possible impact of climate change in the perinatal population. In Aim 1, we will develop an Health Educational Intervention of health promotion and disease prevention services on climate change for the use of perinatal adolescents and their caregivers. The intervention will be delivered through E-zine photos and face to face health education, within an existing integrated mental health (E-zineMH) model in primary care. Aim 2. We will study the feasibility, acceptability, and usability of E-zineMH in a pilot trial among perinatal adolescents and their caregivers in comparison to a control arm that delivers face to face health education in routine antenatal clinics. The findings from this study will provide actionable evidence for technology approach for Health Education Intervention service set-up in primary care in the perinatal adolescent population for testing in a larger study.
