This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. The primary study aim of this trial is to compare the primary outcome as measured by the Schwab and England ADl (PD function), PDQ-39 (QOL), ambulatory capacity (sum of 5 UPDRS questions: falling, freezing, walking, gait, postural stablity), Symbol Digit Modalities (cognative impairment), and Modified Rankin Value (global functioning) in the creatine group versus the placebo group against a background of dopaminergic therapy and best medical care. The research hypothesis is that the group being treated with creatine will show less disease progression by 5 years than the group on placebo as evalutated by a Global Statistical Test comprised of the above measures. Secondard aims are to compare the creaine and placebo groups on additaional measure of efficacy, safety, and tolerability using data between baseline and 5 years follow up.
