This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. Gestational diabetes (GDM) complicates ~4% of all pregnancies in the United States. GDM resembles type 2 diabetes mellitus. Infants born to mothers with GDM are at risk for stillbirth, macrosomia (associated birth injuries), hydramnios and neonatal metabolic complications (e.g. hypoglycemia, insulin resistance, hyperinsulinemia). Mothers are at risk for infections, hypertension, preeclampsia and diabetic ketoacidosis. Tight glucose control reduces risks for mothers and their babies but can be associated with maternal hypoglycemia. Glyburide (GLY) have been used during pregnancy for treatment of GDM however very little is known about the pharmacokinetics (PK) and pharmacodynamics (PD) of this agent during pregnancy. This study will evaluate the PK and PD of GLY during pregnancy as compared to recently diagnosed, matched, type 2 diabetic controls. The hypothesis is that the pharmacokinetics and pharmacodynamics of glyburide are altered during pregnancy. The specific aim is:To determine the pharmacokinetics and pharmacodynamics of glyburide during the third trimester of pregnancy as compared to recently diagnosed, matched, type 2 diabetes mellitus patients.
