Chronic diseases are the leading cause of morbidity and mortality globally and in Australia. While we know of ways to prevent chronic disease, these are often not adopted by services responsible for chronic disease prevention in the community. This CRE will bring together renowned researchers, policy makers and stakeholders, who will work together to develop knowledge, resources and strategies to help community services deliver the chronic disease prevention programs that we know work.
