创伤性骨髓炎是创伤后细菌感染引起的骨组织化脓性炎症,难以治愈且易反复,是创伤领域的世界性难题,治疗骨髓炎的关键在于杀灭致病菌。氢氧化钙(Ca(OH)2)微溶释放氢氧根离子形成强碱性微环境,可杀灭各种致病菌而成功治疗牙髓炎。课题组基于Ca(OH)2的特性制备出新型氢氧化钙骨水泥(Ca(OH)2-PMMA),已证实其具有良好的生物安全性及显著的杀菌效果;结合Masquelet技术用PMMA密封骨髓腔,且其表面形成的诱导膜可促进骨修复重建;Ca(OH)2-PMMA联合Masquelet技术有望成为治疗创伤性骨髓炎的理想方案。项目拟在前期阐明Ca(OH)2-PMMA的理化特性的基础上,进一步探索Ca(OH)2-PMMA的最适宜浓度及应用方案,明确深层杀菌机理及时间-效果关系,系统评估其对局部骨组织及全身的影响,探讨新型材料的诱导膜性能及促骨修复机制,为创伤性骨髓炎的治疗提供充满希望的新思路。

Traumatic osteomyelitis is a suppurative inflammation of bone tissue caused by bacterial infection. It is difficult to cure and easy to relapse, which is a worldwide problem in the field of traumatic orthopedics. The key of treating osteomyelitis is to kill the pathogenic bacteria. Calcium Hydroxide (Ca(OH)2) release OH- to form a local strong alkaline microenvironment, which can kill various pathogenic bacteria and successfully treat pulpitis. Based on the characteristics of Ca(OH)2, we have prepared a new material Calcium Hydroxide Cement (Ca(OH)2-PMMA), which has been confirmed to have good biological safety and significant bactericidal effect. Combined with Masquelet technology, PMMA was used to seal bone marrow cavity, and the induced membrane formed on the PMMA surface can promote bone repair and reconstruction. The combination of Ca(OH)2-PMMA and Masquelet technique is expected to be an ideal treatment for traumatic osteomyelitis. This project intends to further explore the optimal concentration and application scheme of Ca(OH)2-PMMA on the basis of clarifying the physical and chemical characteristics of Ca(OH)2-PMMA in the early stage, to clarify the deep sterilization mechanism and time-effect relation, and systematically evaluate its influence of local bone tissue and the whole body, to explore the induced membrane properties and the mechanism of promoting bone repair, which provide a promising new ideas for the treatment of traumatic osteomyelitis.
创伤性骨髓炎是创伤后细菌感染引起的骨组织化脓性炎症,难以治愈且易反复,是创伤领域的世界性难题,治疗关键在于杀灭致病菌。氢氧化钙(Ca(OH)2)微溶释放氢氧根离子形成强碱性微环境,可杀灭各种致病菌而成功治疗牙髓炎。课题组基于Ca(OH)2的特性制备出不同比例的新型氢氧化钙骨水泥(Ca(OH)2-PMMA),并测试Ca(OH)2-PMMA的表征,系统研究了Ca(OH)2比例对Ca(OH)2-PMMA力学性能、微观结构变化、聚合温度、抑菌性能的影响;构建大鼠动物模型,证实该材料具有良好的生物相容性,能够作为骨缺损植入物促进诱导膜的形成并分泌骨生长活性因子,同时减少了促炎因子的产生,在促进骨缺损修复效果上优于PMMA。Ca(OH)2-PMMA联合Masquelet技术有望成为治疗创伤性骨髓炎的理想方案,为创伤性骨髓炎的治疗提供充满希望的新思路。
