胃癌是我国最常见的恶性肿瘤之一,其治疗效果取决于能否早期诊断。胃癌的早期诊断及手术可以使患者5年生存率提升至84%以上、降低诊疗费用并大幅节省国家医保支出。传统上胃癌的诊断主要依靠内镜检查及病理确诊,新的研究表明Ki67等分子可能成为临床筛查早期胃癌及癌前病变患者的分子标志物。然而我国胃癌早期诊断率不足10%,临床医师对胃早癌体征及分子特征认识不足。因此,研究胃早癌辅助诊断技术意义重大。.本课题将围绕基于多源医学影像及分子特征关联的胃早癌辅助诊断这一问题,对以下三方面开展深入研究:1. 基于内镜影像及病理影像的胃早癌辅助诊断;2. 基于免疫组化分子特征的胃早癌辅助诊断;3. 基于多源影像模型融合的胃早癌辅助诊断。通过上述三个内容的研究,本课题借助人工智能的方法,探究内镜影像、病理影像以及多种免疫组化分子特征与胃早癌的关系,最重建立胃早癌多源数据联合判别模型,以达到精准的胃早癌快速辅辅助诊断。.本课题立足于多学科领域交叉的共性难题,依靠人工智能技术与临床医学影像分析技术,旨在通过交叉研究对胃早癌识别问题进行突破,即高敏感性地对较大工作量的数据进行阳性病例筛查与分类检出。具体的,本课题将建立包含组织病理、胃内窥镜及免疫组化分子特征影像的万级胃早癌多源影像库,同时阐明 AI+医学影像技术在胃早癌辅助诊断问题上的神经网络原理与上层技术应用,为临床肿瘤诊断问题的解决提供新的靶点。从学科交叉角度看,本研究对于胃癌初期诊断、AI+病理技术发展、治疗预后及其适当管理策略的实施等也具有重要意义。
英文摘要
Gastric cancer is one of the most common malignant tumors in China, and the therapeutic effect of it mainly depends on whether it can be diagnosed as earlier as possible. The early diagnosis and operation treatment of gastric cancer can increase the 5-year survival rate of patients up to 84%, so was the decline of patients’ medical expense and national medical insurance expenditure. However, most gastric cancers had been in advanced period by the time it was found, the research data shows that the early diagnostic rate of gastric cancer in China is less than 10%. Traditional diagnosis of gastric cancer relies mainly on endoscopic examination and pathological imaging studies. At the same time, new research indicates that Ki67 may become molecular markers for clinical screening of patients with early gastric cancer and precancerous lesions. However, the outpatient doctors in the primary hospitals have insufficient understanding of the above signs and molecular characteristics,and blind treatment will delay the diagnosis and treatment of early gastric cancer. Therefore, the application of automated image analysis is expected to contribute to the diagnosis of early gastric cancers..This research focusing on the diagnosis of early gastric cancer based on multi-source image and molecular feature correlation includes three parts: auxiliary diagnosis of early gastric cancer based on endoscopic and pathological imaging; auxiliary diagnosis of early gastric cancer based on immunohistochemistry molecular characteristics; model fusion of auxiliary diagnosis of early gastric cancer based on multi-source image and molecular features .Through the above three aspects of research, the method of artificial intelligence was used to integrate different diagnostic information, including endoscopic images, pathological images, and different types of immunohistochemical molecular features of different parts, then a multi-source data joint discriminant model will be established to help diagnose early gastric cancer rapidly and precisely.
