子痫前期(PE)严重危及母胎安全,免疫失衡是其重要发病机制。课题组前期研究发现PE患者肠道菌群紊乱,小鼠移植PE粪菌后出现血压升高等PE样症状,并伴Treg/Th17失衡,提示肠道菌群失调参与PE发病,免疫失衡是其中重要环节,但具体作用机制未见报道。已知外膜囊泡(OMVs)是细菌向外界传递分子效应的主要媒介,前期实验发现PE肠菌来源的OMVs可激活初始CD4+T细胞PI3K/Akt/mTOR信号通路,继而影响Treg/Th17平衡。据此假设,肠道菌群来源的OMVs介导Treg/Th17免疫失衡从而诱发PE。本研究拟从肠道微生物视角出发,以肠道菌群及其分泌的OMVs为切入点,在人体、动物、细胞三个层次,通过宏基组、蛋白组等高通量测序及粪菌移植、囊泡注射等技术方法,深入探讨肠道菌群失调参与PE发病的具体分子机制,可为PE发病机制的研究开辟新思路,并为以肠道菌群为靶标的PE防治策略提供新依据。

Preeclampsia (PE) seriously endangers the safety of mothers and fetuses of which the key pathogenesis is immune imbalance. Previous research of our team has shown dysbiosis of gut microbiome in PE patients and that fecal microbiota transplantation from PE patients to mice models causes imbalance of Treg/Th17 cells accompanied by PE-like symptoms like increasing blood pressure. All of these suggest the involvement of gut microbiome in PE where immune imbalance plays a key role. However, the specific mechanism has not been reported yet. Outer membrane vesicles (OMVs) are known to be the main mediators of bacteria transmitting molecular to the outside world. Pre-test has shown that OMVs derived from gut flora of PE patients could impact the balance of Treg/Th17 cells by activating PI3K/Akt/mTOR signaling pathway in naive CD4+ T cells, according to which we hypothesized that gut microbiome-derived OMVs regulates Treg/Th17 balance in the pathogenesis of PE. Therefore, in this study, we intend to take the gut microbiome and its OMVs as an entry point so as to further explore the specific mechanism of gut microbiome-derived OMVs in PE at human, animal and molecular levels by using high-throughput sequencing such as metagenome, proteome, metabolome, as well as other techniques include fecal transplantation, vesicle injection. The present study could shed light on the novel mechanism of etiology in PE from the perspective of gut microbiome and provide brand-new ideas in prevention and treating strategies of PE in the future.
本课题在国家自然科学基金的资助下,围绕子痫前期(PE)的发病机制展开研究,探讨了肠道菌群及其来源因子在PE中的作用。子痫前期是妊娠期常见的严重并发症,全球发病率为2-8%,但其具体发病机制尚不明确。研究团队前期结果发现,PE患者肠道中阿克曼菌属(Akkermansia)的相对丰度显著下调,同时,嗜粘蛋白阿克曼菌(Akkermansia muciniphila,A.muciniphila)在PE小鼠模型中的丰度也出现明显下降。进一步研究表明,A.muciniphila通过其分泌的外膜囊泡(EVs)能够从肠道转移到母胎界面,显著缓解PE小鼠模型的临床症状,包括血压升高、胎盘结构紊乱和胎儿生长受限等典型表现。机制研究进一步揭示,A.muciniphila来源的EVs通过激活EGFR-PI3K-AKT信号通路,增强滋养层细胞的增殖、迁移和侵袭能力,从而改善滋养细胞功能并缓解PE样病变。为进一步验证其临床应用潜力,本研究还评估了巴氏灭活的A.muciniphila(pAkk)对PE的治疗效果,结果表明,pAkk的疗效与活菌A.muciniphila相当,但其安全性更高,避免了活菌可能引发的感染风险,提供了一种新的、更安全的后生元治疗策略。质谱分析进一步发现,AmEVs中的Amuc_1100蛋白是关键因子,通过抑制TGFβ/SAMD信号通路,减轻滋养细胞功能受损。细胞实验进一步证实,Amuc_1100可恢复滋养细胞的增殖与侵袭能力,并调节相关信号通路的活性。本研究首次系统阐明了“肠-胎盘轴”在PE中的作用机制,提出了以肠道菌群为靶标的PE防治新策略,为PE的防治提供了新的理论依据和研究方向。目前课题组直接及间接研究成果已发表相关论文8篇,培养博士研究生2名,硕士研究生1名,为肠道菌群与妊娠期并发症领域的研究发展做出了重要贡献。
