神经浸润转移(PNI)是胆管癌预后不良的重要原因。肿瘤微环境中胆管癌细胞与神经细胞的相互作用是胆管癌PNI的重要因素,其相互作用机制尚不明确。前期研究发现神经细胞分泌Ach结合胆管癌细胞α7-nAChR,调节胆管癌PNI。结合基因芯片技术发现其可能与lnc01606高表达有关,进一步研究发现Ach与α7-nAChR结合促进胆管癌MUC4及外泌体中lnc01606的表达,lnc01606经外泌体胞吞入神经细胞诱导神经细胞分化并分泌Ach。基于此提出假说:胆管癌来源外泌体介导lnc01606诱导神经细胞分化经Ach—α7-nAChR环路调控“胆管癌细胞-神经细胞”相互作用,促进胆管癌PNI。本课题将采用CHIP-seq、RIP-seq等实验方法,从分子机制、功能表型及临床应用不同层面探索胆管癌PNI的机制及应用,寻找胆管癌PNI的作用靶点,改善患者预后。

Perineural Invasion (PNI) is an important factor of poor prognosis of cholangiocarcinoma. The interaction between cholangiocarcinoma cells and nerve cells in the tumor microenvironment is a significant element in the procedure of PNI of cholangiocarcinoma, however, the specific mechanism is not clear. Previous studies have found that the ACh secreted by nerve cells, which could combine with α7-nAChR of cholangiocarcinoma, own the ability to regulate the PNI of cholangiocarcinoma. Interestingly, in our present study, we found that the procedure of PNI may be related with the high expression of lnc01606 in cholangiocarcinoma through gene microarray. And further studies also verified that the combination of ACh and α7-nAChR promotes the expression level of exosomal lnc01606 and MUC4 in cholangiocarcinoma. Simultaneously, lnc01606 is endocytosed into nerve cells by exosomes to induce the differentiation of nerve cell, which caused more ACh secreted. Based on above researches, a hypothesis is proposed that exosomes derived from cholangiocarcinoma cells mediate the lnc01606 to induce neural cell differentiation and regulate the interaction between the cholangiocarcinoma cells and nerve cells through the ACh/ α7-nAChR loop, promoting the procedure of PNI of cholangiocarcinoma. To test the hypothesis,experimental methods such as CHIP-seq and RIP-seq will be used to explore the mechanism and application of PNI in cholangiocarcinoma from different levels of molecular mechanism, functional phenotype and clinical application, so as to find the targets of PNI in cholangiocarcinoma and improve the prognosis of patients.
背景:胆管癌是一种预后极差的罕见疾病,根治性切除仍然是一种有效的治疗选择。然而可用于术前及预后预测的标志物有限。长链非编码RNA(lncRNAs)在包括癌症进展在内的许多病理生理过程中发挥着重要作用。主要研究内容:对TCGA数据库中有关胆管癌患者癌组织和癌旁组织的lncRNA AL365181.2、ROR1-AS1表达情况的资料进行比较,进行了生物信息学分析,并通过EdU实验、划痕实验、Transwell实验和裸鼠成瘤实验分别检测其对胆管癌细胞增殖、迁移、侵袭和成瘤能力的影响。回顾性分析476例经手术证实的胆管癌患者的临床资料。其中肝内胆管癌(iCCA) 240例,肝门周围胆管癌(pCCA)85例,肝外胆管癌(eCCA)151例。我们利用这些数据进行了统计学分析,以确定影响胆管癌患者术前风险及术后预后预测的因素。对生存组和死亡组进行统计分析。重要结果:与癌旁组织相比,胆管癌组织中lncRNA AL365181.2显著高表达,与癌组织的免疫细胞浸润水平显著相关。ROR1-AS1在胆管癌样本中升高,表达较高的患者总生存期较短。体内外实验结果显示,敲低AL365181.2或ROR1-AS1胆管癌细胞的增殖、迁移、侵袭及体内成瘤能力均显著降低。嗜酸性粒细胞-嗜中性粒细胞比率(NER)和Ca199均为独立危险因素,NER比值和Ca199有显著相关性,最佳截止值分别为36.737和196 U/mL。预测eCCA 周围神经浸润(PNI)的模型变量包括凝血酶原时间、高密度脂蛋白和肿瘤大小,iCCA PNI的变量包括CEA、CA199和肿瘤大小,PNI患者的生存时间明显缩短。预测iCCA微血管浸润(MVI)的变量为淋巴结转移、远处转移、癌胚抗原、肿瘤大小,pCCA MVI的变量为淋巴结转移、癌胚抗原、肿瘤大小,eCCA的变量为淋巴结转移、远处转移、癌胚抗原、肿瘤大小。关键数据及其科学意义:lncRNA AL365181.2、ROR1-AS1可以促进胆管癌的进展,并可能作为诊断和治疗的新靶点。构建的多个诺莫模型具有预测潜力,通过联合NER及Ca199可预测预后,iCCA和eCCA患者术前PNI的预测模型,以及预测iCCA、pCCA和eCCA患者MVI的术前诺莫模型。
