“脂毒性”引起心肌细胞损伤,心脏纤维化和心脏功能减低,是2型糖尿病心肌病的重要病理基础,其发病机制不明。课题组在国家自然科学基金资助下发现,哺乳动物STE20样激酶1(Mst1)在“糖毒性”及1型糖尿病心肌病中发挥关键作用。然而,Mst1是否介导2型糖尿病“脂毒性”诱发的心肌损伤?目前尚不清楚。进一步研究发现,在2型糖尿病状态下,“脂毒性”可能通过促进转录因子FoxO3与Mst1基因启动子直接结合,上调Mst1蛋白表达,继而诱发心肌损伤。然而,Mst1如何介导2型糖尿病心肌病心脏舒张及收缩功能障碍?其分子机制如何?仍需研究阐明。本课题组针对以上问题,拟从基因转录,蛋白表达,蛋白翻译后修饰层面揭示“脂毒性”调控FoxO3/Mst1信号通路,加剧2型糖尿病心肌病的具体分子机制。本项目的成功实施可能阐明2型糖尿病心肌病发生发展的关键分子机制,为降低此类患者的心源性死亡率提供理论指导和新的思路。

"Lipotoxicity" is an important pathological basis of type 2 diabetic cardiomyopathy, which leads to cardiomyocyte loss, cardiac fibrosis and cardiac dysfunction. The underlying specific pathogenesis is not well elucidated. The present research team found that Mammalian Sterile20-like Kinase1 (Mst1) plays a key role in "glycotoxicity" and type 1 diabetic cardiomyopathy. However, whether Mst1 mediates myocardial injury induced by "lipotoxicity" in type 2 diabetes in not clear. Our study found that in type 2 diabetes, "lipotoxicity" may promote the direct binding of transcription factor FoxO3 with Mst1 gene promoter, thus up-regulate the expression of Mst1 and induce subsequent myocardial injury. However, how does Mst1 mediate diastolic and systolic dysfunction in type 2 diabetic cardiomyopathy? The underlying mechanisms need to be elucidated. Our research group intends to reveal the specific molecular mechanism of "lipotoxicity" regulating FoxO3 / Mst1 signaling pathway and aggravating type 2 diabetic cardiomyopathy from the aspects of gene transcription, protein expression and post-translation modification of protein. The successful implementation of this project may clarify the key molecular mechanism of the occurrence and development of type 2 diabetic cardiomyopathy, and provide theoretical guidance and new ideas for reducing the cardiac mortality of such patients.
“脂毒性”(Lipotoxicity)是2型糖尿病心肌病的重要病理基础。阐明“脂毒性”致2型糖尿病心肌病的分子机制,是逆转2型糖尿病心肌病进展的关键。本研究发现(1)糖尿病状态下心脏p-Mst1和Mst1显著升高,p-Mst1/Mst1比值无差异,提示糖尿病条件下心肌细胞Mst1激活主要取决于其转录水平的升高。实时荧光定量PCR,蛋白免疫印迹,染色质免疫共沉淀结果发现,FOXO3在正常条件下与Mst1启动子结合,且在棕榈酸的刺激下结合增强,Mst1响应于高脂刺激转录上调是由FOXO3所介导。(2)超声心动图提示,Mst1敲低改善糖尿病小鼠心脏功能和血流动力学指标。马松染色等提示心脏特异性敲减Mst1减轻心脏纤维化水平。TUNEL染色结果提示Mst1加剧2型糖尿病小鼠心肌细胞凋亡。F4/80和Ly6G组化染色提示Mst1敲减可减轻心脏中性粒细胞和巨噬细胞聚焦。荧光定量PCR结果表明Mst1敲减降低2型糖尿病小鼠心脏炎症标志物水平。(3)体外使用棕榈酸处理小鼠心肌细胞模拟脂毒性造成的心肌细胞损伤,使用腺病毒Ad-shMst1敲减心肌细胞Mst1表达水平。Mst1敲减可抑制脂毒性诱导的心肌细胞凋亡,减轻棕榈酸诱导的线粒体细胞色素C释放,上调线粒体膜电位。此外,Mst1/ JNK1信号通路在糖尿病心肌脂毒性损伤中发挥关键作用。持续活化的JNK1腺病毒转染激活JNK1,可抵消Mst1敲减对于2型糖尿病脂毒性条件下心肌细胞凋亡,细胞色素C释放,炎症反应减轻,心功能改善等作用。本研究揭示了FoxO3/Mst1介导的心肌细胞“脂毒性”是2型糖尿病心肌病进展的关键机制,可能成为糖尿病心肌病治疗的关键靶点。
