基于ICD-11传统医学分类的中医优势病种疾病负担及归因危险因素研究

2018新版《国际疾病分类》（ICD-11）纳入传统医学，是中医融入国际主流医学的重大机遇。其中，国际疾病统计数据中十分缺乏与中医相关的死亡率和发病率，严重限制了中医发展。而综合反映死亡率和发病率最为契合的国际指标是以伤残调整生命年（DALY）衡量的疾病负担，该指标如能成功应用将作为中医对接主流医学的重要证据。本研究拟在全球疾病负担（GBD）理论基础上，分层随机抽样全国3省的中医院为研究现场，筛选出常见中医优势病种，并构建DALY测算模型，实际测算中医优势病种负担并归因危险因素；以消渴病等特定病种探索测定中医特色的失能权重方法；最后以“政策制定科学程序”作为指导性方法，尝试建立我国中医优势病种疾病负担测算体系。本研究理论上创新应用中医流行病学方法，形成基于ICD-11的中医药标准化、国际化示范样本；实践上准确测定中医优势病种疾病负担及归因危险因素，对中医临床与管理具有重要意义。

The 2018 version of "International Classification of diseases" (ICD-11) in the Traditional Medicine(TM) is a great opportunity for Traditional Chinese Medicine(TCM) to integrate into the international mainstream medicine. Most importantly, the lack of mortality and morbidity related to TCM in international disease statistics seriously limits the development of TCM. The best consistent indicator of mortality and morbidity is the disease burden index measured by the disability adjusted life year (DALY). Successful application of DALY will be an important symbol of combine ICD with TCM. On the basis of global disease burden (GBD), this study will randomly and stratified sample Chinese hospitals in three provinces of China as research sites. Then the researchers screen out the usual dominant diseases of traditional Chinese medicine, and construct a DALY calculation model to actually calculate the disease burden of them and attributable risk factors. Meanwhile, they assess the disability and disability weights of Chinese medicine characteristics for specific diseases such as Thirst Disease. Finally, the researchers establish a scientific procedure for policy formulation. As a guiding method, it explores the establishment of disease burden measurement system for dominant diseases of traditional Chinese medicine in China. This research innovates and applies the theory of TCM epidemiology to form a model sample of standardization and internationalization of TCM based on ICD-11. The research results will accurately measure the disease burden and attributable risk factors of dominant diseases of TCM in practice, which is of great significance to clinical medicine and management of TCM.
