乳腺癌经济毒性是指癌症治疗引起的客观经济支出以及由此导致的心理社会困扰，其影响治疗结局和生活质量，已成为乳腺癌照护领域的前沿热点。如何预测乳腺癌经济毒性、如何对经济毒性进行综合干预，机制和模式尚不明确。本研究基于社会生态系统理论，采用量性与质性相结合的纵向混合模式设计，构建乳腺癌经济毒性预测模型和干预策略，分为两阶段。第一阶段为预测模型构建，通过系统评价归纳潜在的经济毒性影响因素，开展纵向研究观察潜在因素与经济毒性之间的关系，应用回归分析构建乳腺癌经济毒性预测模型，并对模型进行内部和外部验证。第二阶段为干预策略研究，通过乳腺癌患者纵向访谈和利益相关人焦点小组座谈，识别社会生态系统各层面对经济毒性管理的供需差距。在此基础上，结合理论框架、系统评价和专家论证，构建乳腺癌经济毒性社会生态系统干预模式并发展配套资源。最后，通过为期3个月的小样本随机对照试验，评价该干预策略的可行性和初步效果。

Breast cancer-related financial toxicity describes the impact of direct and indirect health care costs that lead to significant financial burden for patients and their caregivers, resulting in increased psychosocial distress, diminished patient outcomes, and poorer quality of life. As an urgent issue in cancer survivorship, there is a little evidence about how to predict the risk of financial toxicity and how to take action on it. Therefore, the aim of this study is to establish risk prediction model of breast cancer-related financial toxicity and develop intervention strategies based on social ecological framework. A longitudinal mixed methods design will be used in the study, which consists of two stages. In the first stage, the systematic reviews will be used to identify the potential risk factors. Then, an 18-month longitudinal study will be conducted to observe and estimate the relationship between underlying predictors and the outcome of financial toxicity. Regression analysis will be employed to the establishment of predication model. The internal validation and external validation will be performed to test the accuracy of prediction. In the second stage, a longitudinal in-depth interview with breast cancer patients and several focus interviews of stakeholders will be conducted to explore the gap between the healthcare services and the unmet needs for the management of financial toxicity. On the basis, combining with social ecological framework, systematic literature review and expert meeting, the intervention strategies for breast cancer related financial toxicity will be developed and modified. Finally, a pilot randomized clinical trial will be carried out to evaluate the feasibility and primary effect of financial toxicity intervention strategies.
