视障是导致老年人照护依赖和生存质量下降的严重公共卫生问题，至今尚无理想管控模式。针对其构建与推广中瓶颈问题，课题组基于医院-社区-家庭护联体的体系，借鉴健康管理理论与方法，利用卫生信息技术构建老年人群视障风险VISION管控模式的雏形，即以视障（VI）为核心，确定组织架构（I）、策略与干预（SI）、结局指标（O），标准化流程（N）。本研究应用敏捷开发模式基于混合智能节点模型建立老年人群视障风险VISION智能管控平台；运用结构方程模型分析多中心调研数据找出老年人群视障风险因素并应用半定量方法进行危险度分层；设计老年人群视障风险VISION管控模式的策略与干预以及评价指标；选取试点实证与优化老年人群视障风险VISION管控模式，探寻其实现关键路径，构建出老年人群视障风险VISION智能管控模式，预防和延缓老年人群视障的发生发展，提高照护的可及性，为老年人群疾病管控模式构建与推广提供科学参考。

Visual impairment is a serious public health problem. It leads to the increase of care dependence and decline in the quality of life of elderly people. But there is no ideal management and control model. In view of its bottleneck problem in the building and promotion, based on the hospital-community-family nursing alliance system and referring to the health management methods and theories, health information technology was used to build the working assumptions of VISION management and control model, namely, taking visual impairmet(VI) as the core, identifying organization(I), Strategy & Intervention (SI) and Outcome measures (O)， Normalization process (N). In this study, the VISION intelligent control platform for visual impairment risk in elderly people was established based on the hybrid intelligent node model by applying the agile development model. Structural equation model was used to analyze the multi-center survey data to find out the risk factors of visual impairment in elderly people and semi-quantitative research method to stratify the risk degree. The VISION control model strategy and intervention was designed for the visual impairment of elderly people, and outcome measures were screened. Furthermore, the pilot demonstration and optimization of VISION risk management and control model for elderly people were selected to explore the key path for its realization, aiming at preventing and delaying its occurrence and development, and improving the accessibility of the visual impairment risk management and control of elderly people. It will provide scientific reference for the construction and promotion of disease control work model for elderly people.
