针对PD-1/PD-L1的免疫检查点阻断疗法是NSCLC治疗的重大突破,然而响应率低是其关键瓶颈和科学难点,联合核素内照射的协同策略是解决途径之一。我们前期制备靶向NSCLC肽受体的α核素分子探针(211At-ATE-奥曲肽),发现内照射致肿瘤细胞凋亡或坏死,并增强其免疫原性;同时获得特异性靶向肺癌PD-L1的小分子多肽WL12。基于此,拟利用211At标记WL12,构建新型分子探针211At-ATE-WL12,联合抗PD-1单抗免疫阻断治疗,通过体内外生物学评价和表征分析以及单细胞测序,验证“靶向PD-L1的α核素致肿瘤细胞DNA断裂,进一步激活抗肿瘤免疫应答,从而增益免疫治疗”的协同效应,阐明α核素对肿瘤微环境中免疫细胞和相关信号通路的调控作用,并解析该过程协同增强免疫治疗效果的机制。该研究有望为新型核素分子探针应用于PD-L1高表达NSCLC的临床转化奠定基础,为肺癌治疗提供新思路。

The immunotherapy targeting immune checkpoint PD-1/PD-L1 has achieved remarkable breakthrough of clinical efficacy for NSCLC. However, it has a low response rate, which is the essential bottleneck in its clinical application. Combining immunotherapy and internal irradiation therapy is one of the solutions. We previously constructed an alpha nuclide molecular probe (211At-ATE-octreotide) that targets the NSCLC peptide receptor, and found that internal irradiation can induce apoptosis or necrosis of tumor cells and enhance the immunogenicity. Additionally, we constructed a small molecule polypeptide WL12 specifically targeting PD-L1. Based on this, a new molecular probe 211At-ATE-WL12, combined with anti-PD-1 monoclonal antibodies immune blocking therapy, is constructed to treat NSCLC. Through in vivo and in vitro biological evaluation, characterization analysis and single-cell sequencing, we try to verify that "Alpha nuclide targeting PD-L1 could break the DNA of tumor cells, further activate the anti-tumor immune effect, and thereby increase the synergistic effect of immunotherapy", and clarify the regulatory role of alpha nuclide on immune cells and related signaling pathways in the tumor microenvironment, and analyze the mechanism of combining immunotherapy and internal irradiation therapy. This study is expected to lay the foundation for the clinical application of new nuclide molecular probes in the treatment of NSCLC which overexpress PD-L1, and to provide new strategy for the diagnosis and treatment of lung cancer.
当前PD-1/PD-L1免疫检查点阻断疗法在非小细胞肺癌(NSCLC)免疫治疗中取得重大突破,但由于肿瘤异质性和多种免疫逃逸机制,致多数患者仍无法获益。抑制肿瘤生长、转移、复发以及减轻免疫抑制微环境,是克服NSCLC免疫治疗响应率低的重要研究方向。因此,如何通过精准杀伤肿瘤及调控肿瘤免疫微环境,成为突破这一临床瓶颈的关键科学问题,联合其他抗肿瘤策略是解决途径之一。α核素具备“能量更高、射程更短、更耐乏氧、更易防护”的临床应用优势,为联合免疫疗法解决该问题提供了可能性。围绕α核素靶向治疗促进NSCLC免疫治疗效能提升,我们开展了多方面工作:一是通过高通量基因组学分析,识别并验证了NSCLC演进过程中的8个关键hub基因,特别是GNGT1和NMU基因,表现出一定的诊断和预后价值,为α核素靶向治疗策略设计提供新的靶点。二是设计并开发了一种新型α核素标记方法,大幅提高α核素药物的稳定性、特异性和生物利用率,从而实现对肿瘤细胞的精准靶向打击,大幅提升治疗效果,也为α核素药物的高效标记提供了解决范式。三是系统探讨了PD-L1单域抗体Nb109作为核素标记载体的应用潜能,成功构建了高效靶向PD-L1的核素分子探针,在体内外实验中展现出优异的靶向能力和滞留效果,为α核素靶向治疗的载体选择提供新思路。四是创新性地提出并实施了“阿尔法辐射免疫疗法”新策略,通过构建负载α核素(211At)的仿生化纳米材料,与免疫检查点阻断剂协同应用,形成了“α核素靶向治疗-化学动力治疗-免疫检查点阻断”三模态综合治疗策略,为实现NSCLC的精准、安全、高效以及综合治疗提供行之有效的解决策略。本项目通过多维度、多层次的创新探索,成功构建多种新型α核素药物,并解析α核素靶向治疗增强肿瘤免疫原性、协同增效NSCLC免疫治疗效果的分子机制,为提高NSCLC的治疗疗效开辟了新的研究路径和技术体系。
