The rapid degeneration of cochlear sensory epithelial cells secondary to the supporting cell developmental disorders is an important pathological change caused by GJB2 gene mutation, and its mechanism is still unclear. Previous studies have found that macrophage activation and phagocytosis are involved in the development of several inner ear injuries. We found in the Gjb2 knockout mouse model that the activation macrophages accumulate under the sensory epithelia in the early stage of sensory epithelial cell death. We hypothesized that macrophage activation and phagocytosis are involved in the rapid degeneration of sensory epithelial cells in mice with GJB2 mutations. This study intends to apply space-specific and time-specific Gjb2 knockout mouse models and macrophage differentiation, functional defects, abnormal number of transgenic mouse models, combined with in vitro cell co-culture and cochlear culture, to study the role of macrophage activation in GJB2 gene mutation-related hereditary deafness, and to explore the specific mechanism of supporting cell developmental disorders leading to IRF-7-related inflammatory pathway mediated macrophage activation. On this basis, we further explored the method of reconstructing Cx26 based on maintaining inner ear sensory epithelial cells by intervening in the activation of inner ear macrophages, reducing the phagocytosis of inner ear sensory epithelial cells, and provides a new strategy for implementing mechanism-based GJB2-related hereditary deafness interventions.
GJB2基因突变是导致遗传性非综合征型耳聋的常见原因,感觉上皮细胞及螺旋神经节细胞的级联死亡是GJB2基因突变致聋的主要病理变化。目前,凋亡并不是感觉上皮细胞快速死亡的主要机制,具体机制尚不清楚。基于基因治疗的研究尝试重建Cx26表达,但听力挽救效果有限。维持Corti器正常结构是治疗GJB2相关遗传性耳聋的关键。近期研究发现,内耳存在固有及骨髓来源的巨噬细胞,这些细胞在内耳损伤中发挥作用。在Gjb2基因敲除小鼠模型中,巨噬细胞在感觉上皮细胞死亡前大量聚集,提示炎症反应可能参与GJB2基因突变相关耳聋的发病机制。进一步研究发现应用地塞米松抗炎反应可改善GJB2基因突变相关耳聋。研究提出,GJB2基因突变可能导致支持细胞微管发育障碍,可进一步导致内耳微环境紊乱,介导巨噬细胞活化。该项目通过建立不同GJB2敲除小鼠与内耳毒性损伤模型,发现巨噬细胞活化参与了多种内耳损伤过程,阐明了parthanatos可能是导致内耳毛细胞死亡的主要原因。通过干预巨噬细胞活化及吞噬过程,以保护感觉上皮细胞存活。在此基础上,研究提出通过病毒转染恢复Cx26表达的策略,以期挽救GJB2基因突变导致的耳蜗功能缺陷,为基于机制的治疗提供新思路。
