原发性性腺功能低下症严重影响男性身心健康,而具有睾酮分泌功能的Leydig样细胞移植具有一定的治疗前景。传统的方法诱导干细胞分化为Leydig样细胞不仅耗时长、且效率低。前期实验证实,通过慢病毒过表达NR5A1能使hADMSCs快速、高效地分化为Leydig样细胞,但转基因不安全,很难应用于临床。研究显示,modRNA具有安全、快速的表达特点,而外泌体是一种潜在的天然载体。鉴于此,我们推测外泌体加载modNR5A1将是获取Leydig样细胞的一种安全、有效的策略。因此,本项目拟以人尿源性外泌体为载体,采用优化的超声法加载靶向ac4C修饰的modNR5A1,与自体hADMSCs共培养后,分析睾酮生成及诱导分化后的细胞特性,并探讨靶向ac4C修饰的modNR5A1作用特点。同时结合体内实验,探讨诱导分化后的细胞移植对雄性性腺功能低下症模型大鼠的疗效,为男性性腺功能低下症提供一个新的治疗策略。

Primary hypogonadism seriously affects physical and mental health of men, and Leydig-like cell transplantation with testosterone secretion function has a therapeutic potential. It is time-consuming and inefficient to induce stem cells to differentiate into Leydig-like cells by traditional methods. Our previous study showed that lentivirus-mediated overexpression of NR5A1 could convert hADMSCs into Leydig-like cells rapidly and efficiently, but there are safety problems in transgenic technology, which is difficult to be applied in clinical practice. Recent studies have shown that modRNA has the characteristics of high safety and rapid expression, and exosome is a potential natural carrier. Based on this, we speculate that exosomes loading modNR5A1 will be a safe and effective strategy for obtaining Leydig-like cells. Therefore, for this study, urine-derived exosomes will be used as carrier to load ac4C-modified modNR5A1 by optimized ultrasonic method, testosterone generation and differentiated cells are analyzed after co-culture with autologous hADMSCs, and then the role of ac4C modification is investigated. Furthermore, an in-vivo experiment will be carried out in hypogonadism rat model to evaluate the therapeutic effect of Leydig-like cells derived from hADMSCs. Eventually, it will provide a new treatment strategy for male hypogonadism.
本课题组分别通过CRISPR/dCas9系统介导的基因编辑技术、慢病毒载体介导的转基因、人尿源外泌体(exosome)加载NR5A1(也即SF-1,类固醇形成因子1)的modRNA分子策略,分别成功诱导人包皮成纤维细胞(HFFs)、人脂肪间充质干细胞(hADMSCs)分化为具有睾酮分泌功能的Leydig样细胞。. 我们首先分离了HFFs、hADMSCs作为种子细胞,利用改良后的CRISPR/dCas9系统(CRISPR/dCas9-P300)激活内源性的NR5A1、GATA4及DMRT1基因,成功地使HFFs转分化Leydig样细胞,转分化效率明显优于未改良的CRISPR/dCas9系统;此外,通过慢病毒载体加载NR5A1基因,利用携带NR5A1基因的慢病毒浓缩液感染hADMSCs,实验结果显示:慢病毒感染3天后,细胞形态即发生显著改变,且培养液中能够检测较高浓度睾酮激素的生成。鉴于CRISPR/dCas9基因编辑和慢病毒介导的转基因技术具有不安全性,很难应用临床治疗。. 接下来,为了克服传统方法的缺陷,我们通过利用人尿外泌体替代慢病毒,化学修饰的NR5A1的mRNA(modRNA)替代NR45A1基因,从而大大增加了安全性;通过优化的超声法实现了exosome加载modNR5A1 (Exo-modNR5A1), 与hADMSCs共培养3天,这些细胞形态发生显著改变,经检测具有睾酮合成能力;当这些Leydig样细胞移植到药物去势的雄性大鼠睾丸间质内,这些移植的细胞能存活数周且表达睾丸间质细胞标记物,能够有效提高模型鼠的血清睾酮水平,并建立下丘脑-垂体-Leydig样细胞负反馈轴;此外,移植后模型大鼠的睾丸重量及生精能力也得到明显恢复;通过对比,研究显示:慢病毒介导的NR5A1转基因组和人尿外泌体介导的modNR5A1组的疗效没有统计学差异。本研究通过新方法获取的Leydig细胞具有很强的安全性,具有潜在的应用前景,为临床治疗男性雄激素缺乏症提高了一个崭新的策略。在后续的课题中,我们将进一步优化以提高分化效率、分化细胞的稳定性及持久性。
