复发性流产(RSA)是常见的生殖障碍性疾病,胚胎植入过程中,子宫内膜发生上皮间充质转化(EMT),RSA的发生与该过程减弱密切相关。前期实验发现我院内部制剂养血安胎方通过干预EMT过程对RSA起到关键治疗作用,并引起miRNA-221为主的多种与EMT相关的外泌体miRNA变化。据此,我们提出假说:子宫内膜上皮和基质细胞分泌的外泌体中可能存在多种miRNA参与EMT,并以miRNA-221为主;通过养血安胎方干预外泌体来源miRNA-221靶向转录因子调控EMT,达到治疗RSA作用。故本项目将利用miRNA芯片技术筛选并验证外泌体来源miRNA对子宫内膜EMT的调控作用;利用RNA-seq和ATAC-seq技术通过动物实验和细胞实验阐明养血安胎方通过外泌体促进EMT对RSA的治疗作用,明确养血安胎方通过miRNA-221相关转录因子调控EMT的作用机制,为养血安胎方治疗RSA提供科学依据。

Recurrent spontaneous abortion (RSA) is a common reproductive disorder. During embryo implantation, the endometrium underwent epithelial mesenchymal transformation (EMT), and the occurrence of RSA is closely related to the weakening of this process. Preliminary experiments found that the internal preparation " Yangxue antai formula" in our hospital played a key therapeutic effect on RSA by interfering in the EMT process, and caused a variety of miRNA changes of EMT-related exosomes, mainly miRNA-221. Based on this, we proposed a hypothesis: exosomes secreted by endometrial epithelial cells and stromal cells may have a variety of miRNA participating in EMT, mainly miRNA-221.The therapeutic effect on RSA was achieved through the intervention of Yangxue antai formula to regulate EMT by targeting transcription factors of miRNA-221 from exosomes.Therefore, this study intends to use miRNA chip technology to screen and verify the regulatory effect of exosome source miRNA on endometrial EMT. RNA-seq and ATAC-seq technologies were used to elucidate the therapeutic effect of Yangxue antai formula on RSA by promoting EMT through exosomes through animal experiments and cell experiments, and to clarify the mechanism of Yangxue antai formula's regulation of EMT through miRNA-221 related transcription factors, so as to provide scientific basis for the treatment of RSA by Yangxue antai formula.
研究意义:复发性流产疾病是生殖医学领域的难题,围绕子宫内膜来源外泌体miRNA对复发性流产的病理学影响,探讨养血安胎方的作用机制,为临床应用和推广奠定基础。研究目的:验证养血安胎方通过调控外泌体miRNA影响子宫内膜EMT治疗复发性流产。研究内容:转录组寻找复发性流产患者与健康人群的蜕膜组织存在差异miRNA,挑选有意义的miRNA进行功能验证;从细胞水平验证养血安胎方及小鼠子宫内膜外泌体来源miRNA对子宫内膜上皮细胞EMT的调控作用;从动物验证养血安胎方通过调控miRNA及EMT治疗复发性流产药效研究。研究方法:通过收集复发性流产患者及健康人群蜕膜组织进行smallRNA临床转录组研究,筛选复发性流产血栓前状态疾病患者蜕膜组织中的差异miRNA。通过建立正常妊娠及RSA小鼠模型,提取原代子宫内膜上皮细胞,应用CCK-8、细胞划痕实验、细胞侵袭实验、流式细胞术、Western Blot检测对照组与模型组子宫内膜上皮细胞EMT相关指标。收集原代培养的小鼠子宫内膜上皮细胞分离并观察外泌体。将外泌体添加到RSA原代培养的小鼠子宫内膜上皮细胞中,验证外泌体对子宫内膜上皮细胞EMT的影响。提取正常妊娠及RSA鼠原代子宫内膜上皮细胞外泌体,并对转录组中差异miRNA进行检测,选取有价值的miRNA进行验证。在原代小鼠子宫内膜上皮细胞中过表达及敲低miRNA表达水平,检测其对子宫内膜上皮细胞EMT的影响。通过数据库预测miRNA的靶基因,双荧光素酶报告实验验证其下游的靶基因。制备养血安胎方含药血清,观察其对模型组小鼠子宫内膜上皮细胞EMT的作用。在原代小鼠子宫内膜上皮细胞中敲低miRNA表达水平,同时给予养血安胎方含药血清,观察其对miRNA介导的EMT过程的影响。建立RSA模型鼠后给予养血安胎方治疗,探究养血安胎方通过miRNA调控EMT,最终治疗复发性流产的药理机制。研究结论:养血安胎方通过调控外泌体来源miRNA221的分泌,影响子宫内膜上皮细胞EMT过程,从而治疗复发性流产。
