胃癌前病变(PLGC)是消化系统临床常见病、多发病,且PLGC是胃癌发生的关键阶段。益气养阴活血解毒法临床治疗PLGC效果显著,能从根本上逆转患者和PLGC模型大鼠胃黏膜病理损害。PLGC的发生发展及治疗与肠道菌群和慢性炎症密切相关,有效治疗PLGC是降低胃癌发生率的有效途径,前期研究证实产乳酸菌是PLGC的关键细菌,益气养阴活血解毒法可明显提升产乳酸菌丰度,延缓PLGC进展。短链脂肪酸是其主要代谢产物,NF-κB/NLRP3炎症小体是慢性炎症的关键通路且与短链脂肪酸密切相关。本项目在前期工作基础上,拟以产乳酸菌-短链脂肪酸-NF-κB/NLRP3炎症小体轴为切入点,结合菌群基因组学和代谢组学等方法,探究益气养阴活血解毒法对PLGC菌群组成及代谢产物和NF-κB/NLRP3的影响,阐明其作用机制,为临床应用以调节菌群平衡和控制炎症为靶标的中医药治疗PLGC提供科学依据。

Precancerous lesions of the stomach (PLGC) is a common and frequently-occurring disease of the digestive system, and PLGC is a key stage of the occurrence of gastric cancer. The method of replenishing qi, nourishing yin, activating blood and detoxifying has a significant clinical effect on PLGC, which can fundamentally reverse the pathological damage of gastric mucosa in patients and PLGC model rats. The occurrence, development and treatment of PLGC are closely related to the gut microbiota and chronic inflammation. Treatment of PLGC is an effective way to reduce the incidence of gastric cancer. Preliminary studies have confirmed that lactic acid bacteria (LAB) is the key bacteria of PLGC. The method of replenishing qi, nourishing yin, activating blood and detoxifying can significantly increase the abundance of LAB and delay the progress of PLGC. Short-chain fatty acids (SCFAs) are LAB’s main metabolites, and NF-κB/NLRP3 inflammasome is the key pathway of chronic inflammation and is closely related to SCFAs. Based on the previous work, this project intends to use the LAB-SCFAs-NF-κB/NLRP3 inflammatory corpuscle axis as the entry point, combining the methods of microbiota genomics and metabolomics to explore the effects of the method of replenishing qi, nourishing yin, activating blood and detoxification on PLGC microbiota and its metabolites and NF-κB/NLRP3, clarify its therapeutic mechanism and provide a scientific basis for the clinical application of TCM treatment of PLGC with the regulation of microbiota controlling inflammation as the target.
本研究探讨益气养阴活血解毒法通过调节“产乳酸菌-短链脂肪酸(SCFAs)-NF-κB/NLRP3炎症小体”轴治疗胃癌前病变(PLGC)的机制。实验分为空白组、模型组、益气养阴活血解毒法代表药胃复春组(WFC组)及NF-κB激活剂(BA组)和抑制剂(PDTC组)。结果显示,模型组和BA组胃黏膜炎症、腺体萎缩、肠上皮化生及异型增生评分显著升高,而WFC组和PDTC组显著改善这些病理变化。模型组和BA组胃组织中IL-1β、IL-6、IL-18、TNF-α等促炎因子水平及NF-κB p65、NLRP3、caspase-1、ASC表达显著升高,而WFC组和PDTC组显著降低这些炎症指标,提示益气养阴活血解毒法通过抑制NF-κB/NLRP3炎症小体通路缓解PLGC相关炎症反应。.胃肠道菌群表明,PLGC的发生伴随菌群失调,模型组中厚壁菌门与拟杆菌门的动态平衡被破坏,而WFC组和PDTC组显著恢复菌群多样性,并特异性富集乳杆菌属和双歧杆菌属等产乳酸菌。这些菌群通过促进SCFAs(如丁酸、己酸)的生成,维持肠道屏障功能并抑制炎症反应。SCFAs水平分析证实,模型组中乙酸、丁酸等关键抗炎SCFAs显著降低,而WFC组和PDTC组显著恢复其水平,提示SCFAs代谢在PLGC治疗中的核心作用。.代谢组学显示,WFC组通过调控239种差异代谢物(如脱氧胞苷上调、牛磺次胆酸-3-硫酸酯下调)逆转PLGC的代谢紊乱。KEGG富集分析表明,WFC通过抑制Apelin信号通路、磷脂酶D信号通路等促癌通路,并激活嘧啶代谢、核苷酸代谢等修复通路,实现多靶点代谢稳态调控。网络药理学进一步揭示,WFC的核心成分槲皮素和木犀草素通过靶向调控PIK3CG、MAPK1、EGFR等关键信号节点,显著抑制炎症和促癌通路,同时调节鞘氨醇-1-磷酸、磷脂酸等代谢物的动态平衡。.综上,PLGC的发生与胃肠道菌群失调、SCFAs代谢紊乱及NF-κB/NLRP3炎症小体过度激活密切相关。益气养阴活血解毒法通过上调产乳酸菌、恢复SCFAs水平、抑制NF-κB/NLRP3炎症通路及重塑代谢稳态,有效缓解黏膜炎症、改善腺体萎缩、部分逆转肠上皮化生及异型增生,延缓“炎癌转化”进程。其作用机制涉及“菌群-代谢-炎症”轴的多层次调控,为PLGC的精准治疗提供了重要理论依据。
