抑郁症患者抗抑郁药物治疗疗效个体差异大,机制不明。药物调控脑网络的个体差异及基因多态性均可部分解释抗抑郁药的疗效差异,但不清楚抗抑郁药引起的脑网络动态变化模式及基因、脑网络和疗效的复杂关系。基于项目组前期研究,本项目提出采用影像遗传学研究方法,以未服药抑郁症患者为研究对象,以特异性作用于5-HT的抗抑郁药艾司西酞普兰为工具药开展多时点纵向研究,探索艾司西酞普兰急性期疗效差异的生物学基础。由于既往研究忽略了治疗过程中脑网络的动态变化,本项目将首先通过多个时间点观察急性期治疗期间的脑网络变化,确定药物对脑网络的动态影响及脑网络动态变化与疗效之间的关系;其次,研究5-HT通路关键基因和药物代谢和转运的关键基因在解释疗效差异和脑网络动态变化个体差异中的作用,建立基因-脑-疗效的复杂关系。预期将获得抗抑郁药疗效个体差异机制的新发现,为发现疗效预测的早期生物标记、实现个体化治疗提供理论基础。

Despite dramatic inter-individual differences in clinical efficacy of antidepressants have been observed in patients with major depressive disorder (MDD), the mechanisms underlying the inter-individual variance are not fully understood. The brain networks modulated by antidepressants and genetic polymorphisms can partially explain the inter-individual differences in clinical efficacy of antidepressants in previous studies. However, it is still unknown how brain networks dynamically change during treatment and how the dynamic changes of brain networks link with genetic polymorphisms and the clinical efficacy of antidepressants. Based on our findings in the pilot studies, the current project adopts a longitudinal design, where data is collected during the period of antidepressant treatment with multiple time points among unmedicated MDD patients. The project uses escitalopram as the only antidepressant drug, which is highly potent and selective serotonin reuptake inhibitor. And the project uses imaging genetics methodology to explore the biological bases of inter-individual differences in clinical efficacy of escitalopram during the acute phrase of treatment in MDD patients. This project will make the following contributions. First, as previous studies omitted the dynamic changes in brain networks during treatment, the project firstly investigate the effects of escitalopram on dynamic changes in brain networks by collecting MRI images in multiple time points during acute phrase of escitalopram treatment and determine the relationship between clinical efficiency and dynamic changes in brain networks. Second, this project will investigate the influence of both genetic polymorphisms in 5-HT pathway genes and in genes in charge of escitalopram metabolism and transport on clinical efficiency and dynamic changes in brain networks, and finally build the complex relationship between genes, brain and clinical efficiency in MDD patients. The findings of this project will advance our knowledge about the biological mechanisms of inter-individual differences in the clinical efficacy of antidepressant. This project will thus provide theoretical guidance for identifying biomarkers for efficacy and prognosis evaluation as well as realizing personalized-medicine approaches in MDD.
抑郁症患者抗抑郁药物治疗疗效个体差异大,机制不明。药物调控脑网络的个体差异及基因多态性均可部分解释抗抑郁药的疗效差异,但不清楚抗抑郁药引起的脑网络动态变化模式及基因、脑网络和疗效的复杂关系。本项目构建了包含多个随访时间点的抑郁症患者急性期艾司西酞普兰治疗队列,研究发现:(1)基线期患者的脑功能异常,包括默认网络等多个大尺度脑网络之间的或部分脑区之间的功能连接或效应连接异常、以及局部脑区的静/动态低频振幅异常。(2)患者不同脑功能网络受艾司西酞普兰治疗的影响不同。其中,默认网络核心子网络的网络内功能连接、皮层下网络与腹侧注意网络之间的功能连接、与背侧注意网络相关的效应连接,在药物治疗前后存在显著变化,提示其可能是药物特异性的作用靶点。同时,存在不受药物影响的脑网络连接,比如默认网络背内侧子网络和颞叶子网络的网络内功能连接,在治疗期间没有明显变化,提示其可能是患者的素质性特征,或许可以作为疾病诊断的标志物。(3)存在可以作为预测艾司西酞普兰治疗疗效的影像标志物。其中,情绪调节网络的功能连接、膝下前扣带回皮层的功能连接、双侧距状回/楔叶和右侧舌回/海马旁回两个脑区的低频振幅对患者经艾司西酞普兰治疗后的疗效的预测准确度可以达到72.6%~82.6%。(4)基于抑郁症状的轨迹变化,患者可以被分为“快速缓解组”和“缓慢缓解组”,与这些抑郁症状轨迹变化相关的单核苷酸多态性位点位于染色体2、10和21,并主要参与钙离子信号通路等重要通路,且基于这些位点计算的多基因风险越高,患者治疗无效的可能性越大。总之,本项目已经得到的研究结果为我们深入理解抑郁症的脑功能异常、发现艾司西酞普兰发挥作用的靶点、寻找可用于疾病诊断或进行药物疗效预测的影像指标、以及发现基因多态性与疗效的关系提供了帮助。本项目共发表标注基金资助的学术论文13篇,另有1篇标注基金资助学术论文正在审稿中,预计未来两年可再发表标注项目资助的研究论文2篇。项目负责人与主要成员积极参加国内外学术会议,加强学术交流,已发表会议论文20余篇,参加学术会议30余人次。本项目培养研究生6人,其中博士研究生2人。
