狭窄是食管ESD术后常见的严重并发症,处理棘手。目前治疗效果并不理想,支架是防治狭窄的常用方法,但其再狭窄率超过50%,亟需新型解决方案。申请人前期研发:(1)新型可降解支架,可暂时性解除食管狭窄;(2)新型负载PLGA/曲安奈德缓释微球的京尼平交联壳聚糖胶原贴膜,可使创面愈合的同时抑制疤痕增生。目前体系化联合防治食管ESD术后狭窄的研究鲜有报道。后续申请人拟:①体内外考察新型支架的可降解性,优化抗移位能力;②体内外评价新型载药贴膜的安全性、有效性,优化载药浓度与贴附、顺应能力;③体内二者有机联合应用评价防治ESD术后狭窄的效果及机制,优化应用能力。由此,可获得具有自主知识产权的防治食管ESD术后狭窄的创新成果。本项目基于临床需求,为食管ESD术后狭窄的防治提供新思路,并可推动可降解和载药材料的基础研发与产业化发展。
英文摘要
Stenosis is the main severe complication of esophageal post-ESD. Current treatments are not satisfactory, metal stent implantation is a conventional treatment with more than 50% re-stenosis rate, which needs new solutions urgently. In our preliminary works, we developed a new biodegradable stent having ability to relief esophageal stenosis temporarily without secondary extraction and genipin cross-linked chitoson collagen patch loaded with PLGA/ triamcinolone microspheres which can inhibit scar tissue hyperplasia and not delay the wound healing. As far as we know, there are few reports about systemic prevention and treatments for esophageal post-ESD stenosis. Subsequently, we aim to: Investigate the degradability of the new biodegradable stent in vivo and in vitro, then optimize its anti-displacement ability; Evaluate the safety and efficacy of the patch loaded with drug in vivo and in vitro, and optimize its drug concentration, adhesion and compliance; Evaluate the efficacy and study the mechanism of combined utilization to prevent stenosis of esophageal post-ESD in vivo, and optimize the capability of application. Therefore, we will obtain the innovative method to prevent the stenosis of esophageal post-ESD with independent intellectual property rights. Our research program is based on the clinic needs and provides a new solution for treating post-ESD stenosis, will help to further study the mechanism of re-stenosis and promote the biodegradable and loading drug materials industrialization developments.
