传统免疫抑制剂虽然可以抑制移植排斥反应,但长期使用副作用较大。最近研究表明,由结核杆菌分泌的一种蛋白ESAT-6能抑制T细胞活化。T细胞亚群在参与器官移植排斥和移植耐受中发挥着主导作用,移植排斥主要由Th1/Th17及其效应T细胞(Teff)介导,而调节性T细胞(Treg)则能抑制移植排斥反应并在诱导免疫耐受中起着关键作用。我们首次检测了ESAT-6对移植排斥的抑制效果,初步预实验结果证实,ESAT-6能显著抑制小鼠同种异体胰岛移植物的排斥,并能抑制T细胞的体外增殖与活化。因此,本项目将联合使用ESAT-6与其它免疫抑制剂例如雷帕霉素以诱导移植物的长期存活,进一步探索ESAT-6在移植排斥反应中对T细胞亚群(Th1/Th17/Teff与Treg)分化的影响,并基于TLR2/TRAF6信号通路深入研究其作用的分子免疫机制,为临床治疗器官移植排斥和诱导移植耐受提供实验依据。

Although conventional immunosuppressive agents can inhibit allograft rejection, their long-term utilization usually results in various side effects. Recent studies have shown that ESAT-6, a secretory protein produced Mycobacterium tuberculosis, can suppress T cell activation. As we know, T cell subsets play a major role in allograft rejection as well as transplant tolerance. Allograft rejection and autoimmune responses are mainly initiated and mediated by Th1/Th17 and Teff cells while regulatory T cells (Treg) can suppress allograft rejection and are critical for the induction of immune tolerance. In our preliminary studies, we tested the inhibitory effects of ESAT-6 on allograft rejection. Here we found that ESAT-6 indeed inhibited islet allograft rejection in mice. We also demonstrated that it suppressed T cell proliferation and activation in vitro. Based on these findings, we propose to induce long-term allograft survival by combining treatment of ESAT-6 with other immunosuppressive agents, such as rapamycin. Furthermore, we will study its impacts on differentiation of various T cell subsets, including Th1/Th17/Teff and Treg cells. Finally, we will explore the relevant molecular mechanisms underlying its immunoregulatory effects on TLR2/TRAF6 signaling. This study will provide novel experimental data that support clinical application of ESAT-6 for the suppression of transplant rejection or induction of transplantation tolerance.
本项目将ESAT-6与免疫抑制剂联用以诱导移植物的长期存活,探索ESAT-6在移植排斥反应中对T细胞亚群(Th1/Th17/Teff与Treg)分化的影响,并基于TLR2/TRAF6信号通路深入研究其分子免疫机制。我们首先利用小鼠同种异体皮肤和心脏移植模型证明ESAT-6分别与不同的免疫抑制剂(Rapamycin、anti-CD154抗体和CTLA4-Ig)联用可以延长移植物的存活。ESAT-6抑制供体皮肤和心脏的炎症反应,减少移植物中T细胞浸润。其次利用小鼠皮肤同种异体移植模型,证实ESAT-6减少受体体内Th1、Th17和效应T细胞(CD4+和CD8+ Teff)的比例,减少T细胞T-bet和RORγt的表达。还证实ESAT-6增加皮肤移植物Foxp3+ Tregs的表达和升高心脏移植小鼠体内Treg的比例。体外实验证实ESAT-6抑制Th1、Th17和Teff的分化,促进 Treg细胞的分化并增强其抑制功能,同时抑制T细胞分泌IL-17A、IL-2和IFN-γ,促进IL-10分泌。说明ESAT-6通过调控 Treg/Teff/Th平衡可以抑制移植排斥反应。此外,我们发现ESAT-6在体外抑制T细胞TLR2蛋白的表达和NFkB p65的磷酸化,对TRAF6和MyD88没有明显影响。利用TLR2敲除小鼠建立皮肤移植模型,证实ESAT-6通过TLR2/TRAF6信号通路减少体内Th1、Th17 和Teff的比例,增加Treg的比例,抑制T细胞TLR2蛋白的表达和p65磷酸化。体外细胞实验也证实ESAT-6 通过TLR2/TRAF6通路调控Treg/Teff/Th平衡,为有效安全地抑制移植排斥反应提供临床治疗药物与方案。通过该项目的实施,已经发表SCI收录论文1篇,还有一篇文章手稿正在撰写准备投稿,预计最终共发表 SCI 论文2篇。申请专利一项。培养博士研究生3名,各项指标内容均已顺利完成。
