肝癌切除术后肝功能衰竭与残余肝脏的体积及肝功能密切相关,基于磁共振肝胆特异性对比剂成像对肝功能的定量评估是目前研究热点。既往通过勾画兴趣区的肝脏信号强度及T1弛豫时间对肝功能定量的评估,受技术因素影响较大,结果稳定性差;进一步的研究证实了基于肝脏体素的肝功能定量评估模型对节段性肝功能的准确评估的可行性,但是其忽略了静脉渗出对结果的影响。申请人前期基于肝脏体素的特异性对比剂代谢率-时间校准代谢图,定量评估肝窦阻塞综合征所致肝功能损害。本课题拟进一步建立并验证新的双输入双室双渗出肝胆特异性对比剂代谢模型,精确定量基于肝体素的整体/节段/残余肝功能,指导术前肝切手术方案及预测术后肝衰风险,为术前肝功能定量分析提供准确的影像学依据。
英文摘要
Volume and function of post-hepatectomy remnant liver are closely related to the risk of fatal post-hepatectomy liver failure after resection of hepatocellular carcinoma. Quantitative assessment of liver function prior to hepatic resection based on gadoxetic acid enhanced magnetic resonance imaging(MRI)has not yet been fully elucidated. The quantitative assessment of liver function with T1 relaxation time index and signal intensity-based indices on gadoxetic acid enhanced MRI was greatly affected by technical factors, and the stability of the results was poor. Meanwhile, further studies suggest that segmental liver function based on voxel-derived liver function parameters were performed to explore the inhomogeneous distribution of liver function in patients with liver cirrhosis,but it ignored the effect of venous exudation on the results. Moreover, our previous study has focused on the quantitative assessment of liver disfunction caused by hepatic sinusoidal obstruction syndrome depending on voxel-derived signal intensity-time curve. Furthermore, we will continue to establish a new dual-inlet dual-outlet two-compartment model for quantitative assessment of global/segmental/remnant liver function, to help determine patient prognosis and informthe clinical decision-making procedure.
