童年期状况对老年痴呆发病具有长期而深远的影响,然而相关研究面临着时间跨度长、混杂因素依时变化和影响机制错综复杂等严峻挑战,导致目前研究较少且难以阐明其因果关系。生命历程流行病学理论提出关键期、路径和累积三种假说,试图解释其因果关系。本项目在前期老年健康和老年痴呆调查基础上开展纵向调查,了解童年期状况、老年痴呆等的流行现状和趋势;设定逆概率权重,选取多种预测性能好的机器学习模型,通过交叉验证法训练得到个体学习器,并集成为最优的超级学习组合,估计逆概率权重,构建基于超级学习的边际结构模型;将该模型应用于分析童年期社会经济状况、不良经历和营养状况对老年痴呆发病风险的因果效应,进而探索老年期不良社会关系、心理障碍和健康风险行为因素的中介和调节效应,以验证生命历程的三种假说。本研究将阐明童年期状况与老年痴呆发病风险的因果效应,从生命历程视角为揭示其影响机制奠定基础,为早期防控老年痴呆提供新的思路。

Childhood conditions have a long-term and far-reaching impact on the onset of dementia. However, related studies are faced with severe challenges such as long time spans, time-varying confounding factors, and complicated influencing mechanisms. As a result, there are limited studies and it is difficult to clarify the causality. The theory of life course proposes three hypotheses of critical period, path and accumulation, trying to explain the causality. This project carried out a longitudinal survey based on the prior elderly health survey and dementia survey to understand the prevalence and trends of childhood conditions and the dementia; using super-learning methods, select a variety of machines Learning models with good predictive performance, which are trained by cross-validation to obtain individual learners and integrated into the optimal super-learning combination to estimate the inverse probability weights, construct a super-learning-based marginal structure model; We apply this model to analyze the causal effect of socio-economic status in childhood, adverse experiences, and nutritional status on the risk of dementia, and then explore the mediating and moderating effects of socioeconomic status, adverse social relationships, psychological disorders, and health risk behavioral factors in old age, trying to verify three hypotheses of the theory of life course. This study will effectively infer the causal relationship between childhood status and the risk of dementia, lay the foundation for revealing its influence mechanism, and provide new ideas for early prevention and control of dementia.
本项目背景缘于日益突出的老龄化社会挑战:老年痴呆的患病人数不断攀升,如何从生命历程视角出发,寻找早期干预及预防策略,成为公共卫生领域亟需回答的问题。为此,本项目探索了童年期同伴关系、饥荒暴露及不良童年经历(ACEs)等变量,与老年期的认知功能、老年痴呆等的因果关联,为更好地理解生命历程理论提供了实证支持。.首先,项目组于2023年8月起在晋江市和厦门市社区开展了大规模的老年人失智(失能)调查与干预,通过严格纳入和排除标准筛选出600余名高危老年群体。其次,综合多项全国代表性数据库,采用边际结构模型(MSM)、逆概率加权法、多状态Markov模型等多种因果推断方法,系统评估童年期社会关系、饥荒经历及童年期虐待等因素对老年痴呆及认知健康的长期影响,从而量化并解析了潜在的因果路径。第三,深入探讨了认知功能在童年期经历与老年健康风险间的中介效应,发现童年期同伴关系缺失会带来抑郁、社会隔离及孤独感的叠加影响,进一步加速老年痴呆与失能的发生;而童年期营养不良或饥荒暴露则经由认知功能与心理健康的交互作用显著提升中风或认知衰退风险。第四,本项目构建了多状态Markov模型,分析了老年人认知衰弱(CF)的发生与动态转换,发现认知障碍和躯体衰弱间具有互为因果关系,且部分衰弱状态存在可逆性,这为个性化干预措施提供了科学依据。.研究结果显示:童年期不良经历会在成长过程中叠加并影响成年乃至老年的认知与健康轨迹;反之,童年期积极的社会关系或教育水平较高,往往能带来保护效应。项目共发表学术论文8篇,其中6篇发表于SCI/SSCI期刊,培养研究生4名,完成了预定目标。本项目为从童年期到老年期的“生命历程式”预防模式提供了实证支撑,强调早期社会关系、家庭环境和营养对老年痴呆预防的关键意义,并为公共卫生领域运用因果推断技术提供了可行思路。未来将进一步结合超级学习等机器学习方法,可实现更精准的风险评估与干预策略,为应对不断攀升的老年痴呆挑战提供更具针对性的科学方案。.
