骨折愈合相当于完成胚胎时期骨发育过程。临床上仍有10%的骨折产生延迟愈合或者不愈合,目前尚无理想药物对症干预。我们发现:1)骨细胞是四种成骨因素的靶细胞;2)组成性激活骨细胞Wnt信号提高骨形成和骨吸收,缩短一半小鼠骨折愈合时间,是目前最快的愈合速度,而且产生很小的血痂和骨痂,在抑制骨吸收导致骨折不愈合条件下,也照样愈合,不同于常规愈合。因而提出假说:骨细胞Wnt信号加速骨折愈合。本研究用新构建的模型,实时激活和失活骨细胞Wnt信号,比较正常骨发育,应用自主研发的生物材料与细胞一体化3D打印设备制备骨类器官功能模块,旨在揭示骨细胞Wnt信号快速修复正常的和不愈合的骨折的分子机制。用筛选出的小分子化合物,特异激活骨细胞Wnt,逐步上大动物模型验证其功能。为临床转化提供实验数据,促进骨发育生物学理论和技术进入临床研发阶段。

Fracture healing and bone repair are the courses of skeletal development, similar to that in embryogenesis. However, there is 10% of cases in the form of delayed and nonunion fracture (DFNs) due to diabetes, osteoporosis, smoking, alcoholic, and steroids etc. and there is no approved pharmacological agents aiming at the DNFs. .We found that: 1) osteocytes are the targeting cells responsible for bone anabolism by mechanical stimulation, PTH intermittent treatment, and constitutively active PTH receptor in osteocytes ; 2) constitutive activation of Wnt signaling in osteocytes improves bone formation and bone absorption, shortens a half of time for fracture healing in mice, which is the fastest healing speed at present with the very small hematoma and bone callus. Even under the condition of inhibiting bone absorption leading to nonunion of fracture, it also heals as usual, which is different from conventional fracture healing. .Therefore, we hypothesis that Wnt signaling in osteocytes accelerates fracture healing with specific mechanisms. In this study, we use newly constructed models to activate and inactivate Wnt signal in osteocytes to test the hypothesis, as compared to the bone development under normal condition. Using the self-developed biomaterial and cell integrated 3D printing device to prepare the functional modules of bone organs, aming to reveal the molecular mechanisms by which Wnt signaling in osteocytes accelerates normal and heals nonunion fracture. Using the selected small molecule compounds to activate Wnt signal in osteocytes specifically, verifying its function with larger animal models. The above research will provide experimental data for clinical transformation and promotes the biological theory and technology of bone development into the clinical research and development stage.
基于骨发育的再生医学对骨损伤的修复是国内外骨科领域前沿课题。针对目前尚无批准的药物有效干预骨不连和加速骨折愈合,或者促进闭合性骨折愈合,本研究探索如何加速骨折愈合,以及治疗骨折不愈合的可能性,发现了骨细胞经典 Wnt 信号加速骨折愈合的细胞和分子机制。验证了骨细胞Wnt具有缩短一半骨折愈合时间,且不受骨吸收抑制而产生骨折不愈合的影响。用高通量大数据分析了野生型对照小鼠和骨细胞 Wnt激活小鼠,以及有无注射唑来膦酸共20只小鼠胫骨骨折样本,发现表达差异2倍以上的基因共有831个,其中上调630,下调201。这些基因群包括TGFβ家族的BMP和TGFβ信号,Wnt信号,Notch信号,趋化因子、细胞因子和炎症因子,以及血管、软骨、成骨、破骨等细胞标志分子。分别测定了这些因子的成骨分化功能,用自主研发的 3D 新型打印机构建骨类器官功能模块,快速筛选候选基因,并测试其细胞学功能。发现了骨细胞Wnt和临床成骨药物均能促进转化因子的表达,而且骨细胞Wnt信号、转化因子信号和临床成骨药物均能逆转骨折不愈合。RNA-seq发现大量差异基因富集于脂肪酶活性、脂肪代谢、脂肪细胞分化等与脂肪相关的细胞活动和信号通路中。抑制转化因子信号会促进成脂分化,并在体内、外模型研究中得以验证。发现转化因子信号是成骨分化和成脂分化的开关;Wnt信号与临床骨形成药物均能直接促进转化因子的表达。转化因子能够逆转抑制骨吸收导致的骨折不愈合。申请了多项发明专利,并在撰写论文。. 此外,小分子的发现和应用是构建安全的成骨微环境的策略之一。为了研究Wnt信号激活对骨髓基质细胞分化的影响,小分子药物C91和S33直接作用于骨髓基质细胞ST2,并通过一体化3D打印技术,在三维环境下模拟了小分子药物的体内作用。寻找和构建类似天然骨组织的生理成骨微环境一直是骨再生应用的前沿课题。三种小分子药物S24、C91和S33作用于骨细胞,在3D打印支架中形成了安全有效的成骨微环境,将用于骨折再生性愈合和骨缺损再生性修复。
