慢病控制管理现状不理想,其发病增长快,疾病负担和死因构成逐年增加,严重影响患者生活质量并造成社会及家庭负担。国内慢病的现况调查和健康管理研究较多,但针对糖尿病的监测指标随访分析始终是研究难点且一般采用传统线性混合模型或生存分析方法。这些方法假定指标的线性趋势且数据分布满足特定要求或对基线风险有先验假设,但实际资料常不符合假定而并不适用。因此,探索糖尿病血糖监测随访分析方法是糖尿病健康管理研究中亟待解决的问题。本项目以2型糖尿病患者为研究对象,采用流行病与卫生统计学方法、循证医学和数据挖掘技术,针对复杂的非线性关系、随机效应和交互效应,探索基于神经网络混合效应模型的随访预测建模方法,提高血糖监测随访质量,以利开发合理的监测规则与健康管理策略。本研究为准确预测2型糖尿病随访控糖水平及其并发症提供科学依据、分析方法与循证实践参考,对提高患者生存质量、减轻疾病负担、健康管理等方面具有重要现实意义。

The current situation on control and management of chronic diseases is serious. Its incidence is increasing rapidly with the disease burden and cause of death increasing year by year, which seriously affects the quality of life of patients and causes social and family burden. The health management research on chronic diseases is hot with many available cross-sectional investigations in China. However, the follow-up analysis on monitoring indicators such as blood glucose has always been the research challenge, and it generally used the traditional linear mixed model or survival analysis methods. These methods should assume the linear trend of the data and data distribution meeting specific requirements or some presuppositions of the baseline risks. However, the conditions of the data in practice often do not comply with the assumptions. Therefore, it is a problem to be solved to explore the follow-up analysis methods on monitoring indicators in the study of health management of diabetes. This project is to take patients with type 2 diabetes as the participants, by using the methods of epidemiology and health statistics, evidence-based medicine and the data mining technology, focusing on complex nonlinear relationship, random effect and interaction effect, to explore follow-up monitoring modeling methods on type 2 diabetes based on neural network mixed effect model, which aims to improve the monitoring and follow-up quality of type 2 diabetes, and develop reasonable rules of monitoring and health management strategy. The project is expected to provide a scientific basis, analysis methods and evidence-based practice for complications and monitoring of type 2 diabetes, which has important practical significance on improvement of the survival quality among patients, reduction of the burden of diseases, health management and control, etc.
在全球范围内,2型糖尿病(T2DM)是一种普遍存在的慢性疾病,糖尿病引起的大血管和微血管病变是全世界成人因疾病死亡的主要原因之一,这对糖尿病患者的管理和治疗提出了严峻挑战。本项目首先基于国内外有关2型糖尿病及其并发症管理控制、监测随访与机器学习预测模型的文献进行了综合研究和系统评价。同时,基于既往公共资源研究数据、医院临床资料库等,建立了2型糖尿病及其并发症监测随访多维数据库,并进行了数据预处理、缺失数据插补和数据融合优化。探讨了不平衡糖尿病数据融合下的糖尿病机器学习预测模型构建算法,并对比分析了不同特征变量选择(如Filter、Wrapper和Embedded)和缺失值插补(如多重插补和MissForest)对不同机器学习方法的影响及其在2型糖尿病四种并发症(动脉粥样硬化,冠心病,肾病,视网膜病变)发病风险预测中的应用。Wrapper和Embedded均能有效提升预测模型综合性能,对于动脉粥样硬化和糖尿病视网膜病变的预测,采用多重插补的预测性能指标均表现更好,而在对冠心病和糖尿病肾病的预测中,多重插补与MissForest对预测性能指标的提升各有优势。对2型糖尿病肾病(DN)以及终末期肾病(ESRD)随访数据构建了AVOA-BPNN模型,其模型预测性能优于Logistic 回归(LR)、K近邻(KNN)、支持向量机(SVM)、朴素贝叶斯(NB)、反向传播神经网络(BPNN)等经典机器学习模型。空腹血糖(FPG)的趋势预测对于有效管理糖尿病患者的血糖水平至关重要,对2型糖尿病患者的空腹血糖监测数据,为考虑复杂的个体随机效应与非线性关系,构建了混合效应模型与BP神经网络模型通过Stacking集成的新的组合模型(即混合效应神经网络模型),其模型性能远远优于单一模型。在此基础上,本项目采用R语言编写了相应的模型算法程序,同时给出了2型糖尿病临床数据资料的预处理策略、特征变量选择方法以及2型糖尿病血糖及其并发症监测主要信息预测模型运用的合理规则,从而在糖尿病及其并发症风险预测分析、慢病控制与健康管理等方面具有重要的参考价值。
