进袭性垂体瘤(aggressive pituitary tumors)发病机制不详,缺乏有效的诊疗手段,面临严峻挑战。在前期研究中,我们率先使用单细胞测序技术,通过对五万个垂体瘤细胞的转录组分析,发现在垂体瘤中有一群 “恶性转化潜能的细胞亚群”,而PDZ结合激酶(PBK)在这群细胞中呈现特征性表达。生物信息分析发现PBK的第9位苏氨酸是细胞周期素依赖激酶5(cyclin dependent kinase 5,CDK5)潜在的特异性磷酸化位点。前期实验表明CDK5和PBK可能存在着双向协同磷酸化并导致下游增殖通路失控性激活,而这有可能是垂体瘤恶性转化的关键节点,本研究拟明确如下内容:1)明确PBK在垂体瘤恶性转化中的作用;2)明确CDK5-PBK的磷酸化位点及协同磷酸化在垂体瘤分子分型和预后判断中的作用。期望能从新的视角揭示垂体瘤的发病机制,为垂体瘤的诊断分型及临床诊疗提供有效的分子靶点。

The diagnosis and treatment of aggressive pituitary tumors are confronted with severe challenges, with unknown pathogenesis and lack of effective diagnosis and treatment. In the study, we pioneered the use of single-cell sequencing technology, through the transcriptome analysis of fifty thousand pituitary adenoma cells, and found that there is a sub-group of “cell subsets with malignant transformation potential”, and PDZ binding kinase (PBK) shows characteristic expression in this group of cells. Bioinformatics analysis revealed that threonine 9 of PBK is a potential specific phosphorylation site for cyclin dependent kinase 5 (CDK5). Preliminary experiments have shown that CDK5 and PBK may have bidirectional synergistic phosphorylation and lead to uncontrolled activation of downstream proliferation pathways, which may be the critical node of pituitary tumor malignant transformation. This study aims to clarify the following contents: 1) To explain the role of PBK in malignant transformation of the pituitary tumor; 2) To explicit CDK5-PBK phosphorylation sites and determine synergy phosphorylation in the role of the molecular classification of pituitary adenoma and prognosis judgment. It is expected to reveal the pathogenesis of pituitary tumors from a new perspective and provide a valid molecular target for the diagnosis and treatment of pituitary adenomas.
垂体腺瘤是神经系统肿瘤中较为常见的一种,约占颅内肿瘤的10-15%,其中进袭性垂体瘤(APTs)表现出对邻近结构的侵袭性,如海绵窦、颅底斜坡等,肿瘤快速生长,复发风险高,对标准疗法反应不佳,给患者带来极大困扰。本研究聚焦于CDK5与PBK之间的相互作用及其在垂体瘤侵袭性发展中的关键角色。研究发现,CDK5与PBK能够直接结合,并在PBK的T9位点以及CDK5的S159位点上实现相互磷酸化,形成了一种独特的相互磷酸化机制。本研究系统探讨了CDK5磷酸化PBK-T9磷酸化通过P38 MAPK 通路垂体瘤细胞中促进增殖和侵袭的作用。明确了PBK-T9磷酸化在进袭性垂体瘤中发生发展的关键作用。本研究验证了pPBK-T9高表达于进袭性垂体肿瘤中,与肿瘤侵袭显著相关,是肿瘤侵袭的独立风险因素。上述结果验证了我们提出的科学假设:CDK5通过磷酸化PBK-T9促进垂体瘤的进袭性生长。进一步的实验表明,在侵袭性垂体瘤组织样本中,磷酸化的PBK-T9和CDK5-S159表达水平显著升高,且两者的共表达与肿瘤的体积及侵袭性呈显著正相关,表明CDK5与PBK之间的相互作用在垂体瘤的侵袭性进展中扮演着至关重要的角色。此外,本研究还验证了CDK5-PBK相互作用对下游Insulin信号通路的影响,该通路对垂体瘤的增殖进展具有显著的调控作用。通过应用人工智能技术,筛选出针对CDK5和PBK的双重靶点抑制剂,并在分子生物学实验中证实了其对CDK5和PBK的双靶点抑制效果,以及对垂体瘤增殖进展的有效抑制作用。本研究不仅深入探讨了CDK5与PBK相互作用的分子机制,还为垂体瘤的个性化诊疗提供了潜在的分子靶点,有望推动垂体瘤治疗策略的进一步发展。
