神经根型颈椎病(CSR)是临床多发病,推拿手法疗效显著。前期研究发现颈椎斜扳法治疗CSR即刻症状可减轻,即时效应明显。目前中医手法显效的关键机制仍不清晰,即时效应机理是亟待解决的科学问题。以往对中医手法的机理研究主要集中在椎间盘及椎体应力等方面,尚无针对神经根与周围组织位置即时变化及椎管有效横截面积的研究。在前期研究基础上,提出假说:颈椎斜扳法可以即时改变神经根与周围组织位置关系,神经根在手法作用下可躲避刺激;手法能增加椎管有效横截面积,减轻神经根周围的张力。本课题拟运用动作捕捉技术采集颈椎斜扳法的力学参数、运动轨迹,借助3.0T超导核磁共振获得背景抑制弥散成像(DWIBS)及弥散张量成像(DTI),分析病变神经根与其邻近组织位置关系,观察椎管有效横截面积变化;继而采用有限元技术分析病变部位应力及应变,综合解析手法改变责任神经根与椎间盘、椎间孔等结构位置关系,阐释颈椎斜扳法即时效应机理。

Cervical spondylotic radiculopathy (CSR) is a clinical frequently occurring disease, and the therapeutic effect of traditional Chinese medicine (TCM) of manipulation is significant. Previous studies found that, the symptoms of CSR could be immediately alleviated through manipulation. However, the mechanism of immediate effect of TCM manipulation is still unclear. Researches on the mechanism of TCM manipulation mainly focused on the change of stress in intervertebral disc and vertebral body, but there was no research on the real-time change of nerve root and surrounding tissue position and the effective cross-sectional area of spinal canal. On the basis of previous study, we hypothesized that the immediate symptoms of CSR can be alleviated by the treatment of cervical inclined wrench manipulation, which is due to the improvement of the relationship between the nerve root and the surrounding tissues and the increase of the effective cross-sectional area of the spinal canal. In the present study, motion capture technology is used to collect the mechanical parameters and motion track of cervical inclined wrench manipulation, with the help of diffusion weighted whole body imaging with background body signal suppression (DWIBS) and diffusion tensor image (DTI) were obtained by 3.0T superconducting MRI. Combined with conventional and neuroimaging sequences, the relationship between the pathological nerve root and its adjacent tissues will be analyzed, and the change of effective cross-sectional area of spinal canal will be observed. Then, the stress and strain of the pathological site will be analyzed by finite element method, and the responsible nerve root, intervertebral disc and intervertebral disc will be changed by comprehensive analysis. Our study will provide foundation for mechanism of immediate effect of cervical inclined wrench manipulation.
