多发性骨髓瘤(MM)是起源于B细胞系的恶性血液系统疾病,其发生发展主要取决骨髓微环境和细胞内部的基因不稳定性。炎性细胞及炎症因子作为骨髓微环境的组成部分,可通过诱导基因不稳定性促进MM发生发展。我们的前期研究发现,MM患者血清促炎性因子IL-32水平明显增高,后者是炎症反应“级联放大效应” 的关键因子之一,促进骨髓基质细胞分泌IL-6等炎性因子;而IL-6是 MM 细胞增殖最重要的细胞因子,与MM的发病密切相关。本课题拟采用细胞学、免疫学、分子生物学、动物实验等方法,在体内外研究IL-32及其触发的IL-6等因子分泌的作用及其具体机制,观察IL-32作用下细胞因子网络的改变以及该网络中各炎性因子对MM细胞基因不稳定性、细胞增殖的作用和分子机制;旨在阐明炎性因子网络在MM疾病病理机制中的作用,为早期干预MM发生、发展提供理论基础,为进一步设计新的靶向治疗策略提供实验依据。

Multiple myeloma (MM) is a malignant blood system disease which originates from the B cell lineage, its development mainly depends on the bone marrow microenvironment and genetic instability of the cells. Inflammatory cells and inflammatory factors, as parts of the bone marrow microenvironment, can promote the occurrence and development of MM by inducing genetic instability. We have found elevated serum levels of inflammatory factors IL-32 in MM patients, the latter is one of the key factors in inflammation ‘cascade amplification effect’ promoting secretion of inflammatory factors such as IL-6 by a variety immune cells. IL-6 is the most important cytokine in MM cell proliferation, closely related to the occurrence of MM. We propose cytology, immunology, molecular biology methods and animal experiment, to study the role and specific mechanism of IL-6 and other inflammatory factors triggered by IL-32 in vivo and vitro, to further observe the change of cytokine network under the action of IL-32, to find the effect of inflammatory cytokines in MM cell genetic instability and proliferation, apoptosis, drug resistance and their molecular mechanism. To clarify the role of inflammatory factor network in MM disease pathological mechanism, providing theoretical and experimental basis for early intervention and further new targeted therapeutic strategies of MM.
在MM中,炎症与其发生发展关系密切。我们前期研究发现,MM患者血清中促炎因子白细胞介素32(interleukin 32,IL-32)表达水平较健康对照明显增高,但其在MM微环境中所发挥的作用也尚未明确。Cytokine array以及ELISA结果显示,MM 患者外周血血清中以及骨髓上清中IL-32 的表达均较对照组明显增加(p﹤0.01)。2. RT-PCR、Western blot以及ELISA检测提示,MM细胞株(RPMI 8226、OPM-2)中IL-32的表达较骨髓基质细胞(BMSCs)明显增加,但是PR3(IL-32受体)的表达明显降低(p﹤0.01);骨髓单个核细胞(BMMCs)经CD138磁珠分选后,RT-PCR结果以及ELISA检测上清均提示,CD138+细胞中IL-32的表达较CD138-细胞升高(p﹤0.05)。3. Cytokine array提示,外源性重组IL-32(rIL-32)刺激BMSC后,IL-6、MIP-1α、MIP-1β等明显升高;ELISA 验证并检测了发现rIL-32刺激后其IL-6分泌的上升呈浓度时间依赖性(p﹤0.05);用Sh-RNA干扰IL-32表达后的MM肿瘤条件培养基培养BMSCs,ELISA检测发现IL-6的分泌较对照组下降(p﹤0.05);Si-RNA干扰BMSCs中PR3表达后加入rIL-32刺激,ELISA检测发现IL-6的分泌较对照组明显下降(p﹤0.01)。4. rIL-32刺激MM细胞株后,MTT法检测细胞增殖,流式细胞术检测细胞凋亡,发现其与对照组相比均无显著性差异。5. rIL-32刺激BMSCs后,Western blot检测炎症因子相关通路NF-κB、JAK-STAT3等,发现其磷酸化水平较对照组明显上升;Si-RNA干扰BMSCs中PR3后该变化减弱。此外,我们的研究还发现IL-32通过激活STAT3和NF-ΚB通路诱导MΦs向M2型极化,从而调控MΦs诱导MM耐药的过程,为克服MM耐药提供新的靶点和实验基础。在MM瘤骨髓微环境中,IL-32主要来源于CD138+肿瘤细胞,它通过与BMSCs表面的PR3结合,激活下游的NF-KB、JAK-STAT3通路,刺激其分泌IL-6以及其他炎症因子、趋化因子等,进而促进MM肿瘤细胞的增殖、趋化等。
