应力性骨折即疲劳骨折是常见的运动系统损伤。目前对其发生病理过程有初步认识,已知骨细胞对力学信号感知与反应在其中发挥重要作用,但详细机制不清。在前期研究中发现,骨细胞中的FGFR1可能参与其力学感知及功能活性的调节,且缺失FGFR1的骨细胞存在细胞重要感受器-纤毛的异常。我们提出FGFR1可能经JNK1调节Dvl2/Wnt信号及纤毛长度与功能,进而影响骨细胞的功能与活性,参与应力性骨折的发生与修复。本课题拟利用骨细胞中增强/敲除FGFR1小鼠,建立力学加载模型,通过骨骼表型分析及细胞培养结合JNK1、Dvl2相关干预措施,深入研究FGFR1经调控纤毛及骨细胞功能在应力性骨折发生、发展中的作用与机制。本研究可望明确FGFR1参与骨细胞功能维持及应力性骨折发生与修复的新机制,为探索应力性骨折的防治措施,以及为纤毛疾病的机制及治疗策略研究提供借鉴。

Stress fracture, also known as fatigue fracture, is one of the most common injuries of skeleton system. To date, there is relatively less understanding of its pathological process,especially the underlying mechanism. The sensing and response of osteocytes to mechanical stimuli play an important role in the pathogenesis of stress fracture, but the detailed mechanism is largely unknown. In recent study, we found that the FGFR1 signaling in osteocytes may be involved in the regulation of its functional state and mechanical perception, and the osteocytes lacking FGFR1 have abnormalities of cilia, a complex signaling center and sensor. We thus propose that FGFR1 may regulate the function of osteocytes via JNK1 mediated regulation of Dvl2/Wnt signaling, and participate in the pathogenesis and repair of stress fractures. To reveal the detailed cellular and molecular mechanisms, we will combine mice with genetically modified FGFR1 in osteocytes with cell culture and JNK1 and Dvl2 related intervention reagents in vitro to study the roles and mechanisms of FGFR1 in the regulation of stress fractures through primary cilia. This study is expected to clarify the novel mechanism of FGFR1 signaling involved in the maintenance of osteocytes and the pathogenesis of stress fracture, providing an experimental basis for exploring the prevention and treatment of stress fracture, and studying the mechanism and treatment strategies of other ciliopathies.
针对应力性骨折(即疲劳骨折)这一常见的运动系统损伤,探讨其发生和修复的病理过程。已有研究表明骨细胞在力学信号的感知与反应中起重要作用,但其详细机制尚不明确。我们前期研究发现,骨细胞中的成纤维细胞生长因子受体1(FGFR1)可能在力学感知及功能活性调节中发挥关键作用,且缺失FGFR1的骨细胞表现出功能活性异常。据此提出假设:FGFR1可能参与调节骨细胞对力学信号的反应,从而影响应力性骨损伤的发生及其后的组织修复。本课题首先通过动态轴向加载诱导小鼠右侧尺骨微骨折,建立了稳定的应力性骨折模型,并解析了其修复病理过程。研究发现,应力性骨折的愈合主要通过膜内成骨模式进行,无软骨生成。同时,新生编织骨区域的神经分布密度在损伤后呈先上升后下降的趋势。通过RNA测序(RNA-seq)数据分析,发现fgfr1和gli1在损伤后7天高表达,随后逐渐降低,且两者FGFR1和Gli1在新生编织骨存在共定位,提示FGFR1可能在损伤修复早期发挥重要作用。进一步通过基因修饰小鼠和相关阻断剂干预研究,发现FGFR1可通过PI3K-AKT-mTOR信号通路调控Gli1阳性间充质干细胞的活性,参与应力所致的骨损伤修复。针对神经调控在应力损伤修复中的作用,研究发现6-羟基多巴胺(6OHDA)介导的交感神经切除可促进应力性骨折的愈合,而辣椒素去感觉神经化则延缓应力性骨损伤的恢复,Gli1阳性的骨骼干/前体细胞参与该过程。低强度脉冲超声通过激活Piezo1信号促进应力性骨折的结构及力学性能修复。本研究明确了FGFR1在骨相关细胞功能维持及应力性骨折发生与修复中的关键作用,为探索应力性骨折的防治措施提供了新的理论基础和研究借鉴。
